Hello I'm 30M experiencing persistent difficulty reaching orgasm during partnered sexual intercourse.
During sex, I am able to become highly aroused and often reach a point where I feel I am very close to orgasm. However, I frequently get “stuck” at this high level of arousal and I am unable to progress to ejaculation. It feels like I reach a peak sensation but cannot cross over into release.
A key pattern I have noticed is that my mental state strongly affects this process. If my thoughts shift, I become distracted, or my focus changes even briefly, I tend to lose the arousal progression and the sensation drops, making it difficult or impossible to finish. I feel I need to maintain a very specific and continuous mental focus and emotional state in order to reach orgasm.
I also notice a clear difference between solo sexual activity and partnered sex. I am generally able to reach orgasm more easily when alone, while the issue is more consistent during sex with a partner.
This situation sometimes leads to sexual encounters lasting a long time (often over an hour) without ejaculation, which can cause stress and concern for both me and my partner.
I would like to understand the possible psychological or physical causes behind this, including factors such as performance anxiety, attention/focus issues, stress, sexual conditioning, or pelvic floor tension. I am also interested in effective treatment approaches or strategies to improve this situation.
I am seeking guidance on how to resolve this issue and improve sexual function and consistency.
