Nicholas Lawn and Elaine Pang: they gave us a problem, without solution

I'd prefer never have read The Lawn and Pang study about nocturnal seizures, after a unprovoked seizure.

They basically say:

  • 57% Will recur in 1 year
  • 77% Will recur at 10 years
  • EEG and MRI dont change recurrence rate
  • ASM doesnt change recurrence.

Then... They gave us a problem, without solution.

You basically must accept your fate and thats It.

There's no Hope? How The medicine can be too useless against some conditions

Anyone already experienced this?

I Woke up very confused, heart racing, with a Lot of pain in my upper back muscles, and i noticed The bed was wet. Ive urinated on It.

I had drooled a Lot.

My mom told me i was with apparent difficulty to breath, then SHE turned me to The side.

I then standed up , tried to put my Flip flops, and went to bathroom. My dad tried to hold me, and i pushed his arms and Said "get out".

I have panic disorder for 20 years and GAD. but at this time i think It was a seizure or something like that.

What are your triggers? And how effective is avoiding them?

I was wondering about how The ILAE and scientists in overall "ignore" The triggers and precipitating factors.

There are too few studies about recurrence risk, frequency, reduction in frequency when avoiding triggers.

There are officially 2 "types" of classifications of seizure: provoked or unprovoked

But between them, i guess there are many possibilities, like The cases occuring only with triggers, or some precipitants.

Do you guys have any Idea on How avoiding or exposing yourselves to these triggers affect The seizures?

I know that there are many people that dont have any obvious/possible factor and still have seizures. But also there some people that cannot have alcohol, or bad Sleep, or high stress/hyperventilate..

Teorically when you know The triggers, avoid your triggers, and use meds, you are greatly protected.

Anyone got Nocturnal seizures freedom?

I read a study (Lawn. 2023) that 57% of people with First unprovoked seizure from Sleep recur within 1 year.

And 77% in 10 years.

To make It worse, they say in study that those who received AED had The same recurrence rate to those that dont get meds.

Well, i know that ITS Just one study, with less than 300 inpatients, but ITS scary.

Then i Wonder if there are some people here who got freedom from The nocturnal seizures?

Lamotrigina, anyone tried?

Any experience with lamotrigine for anxiety/panic/GAD?

I was reading that ITS not useful in anxiety, but some psychiatrists and neurologists prescribe for these cases.

How you guys deal with having to work and the fear of having a seizure?

Home office tends to be easier, but what about having to Go out, taking bus/Uber and staying a Full day in an office?

The studies show that a lot of people with Epilepsy have anxiety and panic disorder.

But what came at first? Ep or anxiety/pânic?

Some researchers also say that people with panic disorder/GAD could have a lower threshold because of disfunction at GABA "system" and glutamate availability.

Joëls, M. et al (2009) showed how stress and anxiety hormones Disturbs directly the cortical excitability.

Kanner, A. M. (2011) showed that panic disorder and anxiety share the same hyperexcitabilty mechanism of that of Epilepsy. He showed people with anxiety and panic disorders have 3 to 7 fold more risk to develop seizures and Epilepsy.

The study argues that specific stress (such as panic) causes hyperactivation of the HPA axis, resulting in structural changes in neurons that exceed convulsive limits.

This is an interesting topic. I wonder If there are many people here with panic and anxiety disorder diagnosied Before ep?

I simply cannot do anything anymore..

Cannot work, possibly losing my job. Fear to go out

Stopped biking (that i love to do), fear of driving..

Stopped talking to people except my parents.

And i not mentioned The absurd and most terrible part that is having MRI and waiting for The results (almost got Crazy in anxiety and fear).

Well, any hints about how to deal with this?

Im really not resisting.. thinking really about giving up

I have panic disorder and GAD since i was 15 (now in 36), and always had too much fear of having a seizure.

So last month i went too Sleep and Woke UP with paramedics over me.

Heart racing, sore muscles in upper back and TMJ, bedwetting and my mom told me i was drooling when she came to see me.

SHE told me i walked to The bathroom, tried to put Flip flops and acted like "sonambulism", but Ive read It could be the POST ictal phase.

SHE was scared and then Called 911/ambulances.

Well, since that day im having more panic attacks, my anxiety multiplied, and i simply cannot live anymore. Stopped working. A Lot of depression, fear of sleeping. ITS a nightmare.

This was simply one of The most terribles nightmares becoming true.

I still cannot accept this. After years of giant fear of seizures, i ve Woke UP weeks ago drooling, with bedwetting, a Lot of pain in my upper back, with paramedics around me, and went to Hospital.

My mom told me she found me at this state, with opened eyes. Then i started deambulating, tried to put Flip flops, went to bedroom, pushed my father arms..

I probably had a seizure. There are some doubts about PNES or parassomnia with possible apneia, because one can mimick another.

Since from that day i simply cannot live anymore. My GAD and panic syndrome came back with Full strenght. Cannot work anymore, fearing Sleep, fearing going out (even more)

I never thought that one of my biggest fear would happened, and It probably did. Now ITS being too hard to deal with this.

Ive read that people with cronic anxiety, like GAD, panic syndrome, bruxism, migraine aura, bad Sleep, Nocturnal panic attacks, paroxystic vertigo, Restless legs syndrome, chronic stress, Hipocondria/hypervigilation, ADHD, hypnagogic Sleep events, could have a "hyperexcited Brain", but never seen people with anxiety and those things having this.

Has anyone else experienced something similar? Really hard to accept this.

PS: I use IRSR (Sertraline) for 10 years, and i was using It Very irregular weeks before The event. Like 100 mg one day, 0 another, 100 again, then 50, 0, 0, something like that. I was having bad Sleep fragmentation, waking up many times..

I've just read some studies that say that The recurrence of seizures when you had first when sleeping is 57% at 1 year and 77% at 10 years.

And people with medication didnt reduce The recurrence rate.

Well, basically they are saying that there's nothing to do.

You Just have to accept that you need a permanent caregiver, cannot Sleep/live Alone, and Will live forever knowing that you can have this sh1t anytime, especially in Sleep.

Anyone here ever found a way to become seizure free or reduce drastically The recurrence rate?

Its so sad that medicine still cannot do anything effective against nocturnal seizures.. oh i know that surgery can be made in some people, removing parts of The temporal lobe, but thats too risky and doesnt work for ALL.

But there arent any meds effective against that? This is Very frustrating. Ive been not working. Very depressed. I got no life anymore
