Early termination

I am currently on deferred adjudication and just left my PO talking about early termination. My PO said I can get a form off google and submit it after I fill it out. Is there anybody that’s done this before and has a good form or link to one? Idk if I’m over thinking how “professional” it needs to look but nothing I find online looks right to me. Thanks

Arlo

This is Arlo! We’ve been assuming he is a Lab/Chow mix. The mom is pictured. She was found pregnant and had puppies. Some with her coat, some that are light brown and then a single black pup. Idk if that’s very normal? But we went with the odd ball! Anyway, opinions on if he’s actually mixed with a black lab?
