Whats missing on this bike or what could make this bike even better?

A bit of a odd situation. I am a NEXUS holder and every time i try to use the machines at Canadian airports i get errors and my passport does not scan properly (its less than 5 years old). I was told by the Nexus officers that my passport chip is not working properly. This has also come up when traveling to the EU recently and once in Mexico.

How exactly do i get a replacement? all of the forms regarding changes are based on bio data info but when it comes to a bad chip... im not sure i know what the process is, not only that because the passport still has more than 1 year left of validity, i cant just "renew" it or can i?

Any help on how to proceed here is appreciated.
