Weight: 67kgs

Height: 5’4”

Current issues:

1/ Severe PMS symptoms: extreme low mood, irritability, back pain, cramps, body ache which starts almost 7-10 days before period date. Regular cycle of 30 days.

2/ Heavy flow during periods and extreme cramping and pain which does not reduce with any pain killer, heating bag, or anything else. Only cure that works most of the time is drinking the water of boiled coriander seeds (boiled for 25 mins).

3/ Gastro: swings in appetite from high to low. Low days feel like an intolerance to food, nothing feels appetising and the thought of eating is nauseating. High days feel like my stomach is a black hole and I could eat anything and everything. Episodes of burning sensation in the gastro system immediately post swallowing of food — even when on a healthy diet.

4/ Severe episodes of allergic reactions, often induced by stress and often due to no apparent reason. All allergy tests are negative. History of severe atopic dermatitis in mother.

Past issues:

1/ As a child, I used to have persistent stomach aches which were recurring and no cause was ever determined by doctors. Pain episode has been so high that I was given an injectable pain relief when I was 16. This issue now doesn’t exist as much and rarely appears. It just randomly stopped. Right now, the only thing that induces a similar pain in the abdomen is if I’m standing for too long. Walking does not cause it.

2/ History of anxiety, ED, and currently under treatment for MDD.

Family history:

Father: high blood pressure, high eye power, history of gall stone

Mother: atopic dermatitis, gastric ulcers, extremely sensitive skin that reacts to almost all unnatural stimuli, depression, anxiety

Brother: slight hypochondria (undiagnosed)

Grandparents: Schizophrenia, brain stroke, heart attack, depression, diabetes, severe obesity

A friend needs help in getting a diagnosis

She is 28 F from India and has recently been extremely fatigued, so fatigued that she is tired with some house chores or walking for a few minutes. This is going to be a really long post but she really needs someone to tell her what is going on with her. It all started last year around September, she puked and had very bad abdominal pain. Then she got a fever of 104 degrees Fahrenheit and went to emergency. She was given antibiotics, which cured her fever but abdominal pain never went away. In the coming days, she randomly had blood in urine and blood in stool for a day or two. All tests done, nothing came up. Ultrasound showed nothing, stool tests showed no blood later as it was just for a day or two. She got endoscopy and biopsy and she got nothing in endoscopy. Biopsy showed duodenum partial villous atrophy. Colonoscopy showed nothing. Till now her abdomen pain hasn't gone. She has been tested for all possible test for celiac and they were negative. Since almost 8 months she has had this abdomen pain. She has taken drotin, meveberine and both of them did not help.

Recently her fatigue has increased. She gets hot flashes or cold sweats sometimes and usually at nights. Her iron levels were recently replenished with an IV and she even got a Vitamin D shot. She thought her fatigue was bad because of deficiencies but even after iron IV she feels really bad fatigue. Currently her B12, folate, iron and Vitamin D, all are good. Her ferritin is also really good. No deficiencies. She also gets migraines and has back issues. She has depression and anxiety but is on meds for that and they are not giving her any trouble. Her libido has also dipped in past few years.History given below.

History

She has had this pain since childhood. It worsens when she stands for some time. It used it get so much worse that she would either puke or almost faint. As a kid always had abdominal pain issues. She remembers having this as young as 8-9 years old. Even when she would get hurt on her hand or foot, she would first feel extreme pain in her stomach. As a kid she had headaches and bruised easily, she had random blue green bruises on her legs specially. She has a history of PCOS (now PMOS) but now she has no cysts and regular periods. She has had bad fatigue since childhood. She remembers having really bad leg pains as young as 7 years old. She is lactose intolerant and has always had gastric or acidity issues. In 2019 once she had an episode of puking 10-13 times in 2-3 hours. Doctors thought it was intestinal inflammation and was resolved with ayurvedic medicine as allopathy didn't work for her. She randomly had these puking episodes where she randomly pukes once and then has bad abdominal pain every few months, every year. She also gets recurrent UTIs. Last year she had recurrent HSV-1 episodes with severe allergic reactions and fungal infections and no one around her got it.

In 2021 she had severe neck pain and MRI revealed her neck's curvature was reversed. Same thing happened with her back spine in 2024 and she took supplements and her pain resolved and the doctor just suggested this. She had discussed bulge issues. She has no injury history which could have resulted in this. Once her jaw dislocated randomly as she was chewing her food from only one side of her mouth as she had a broken root canal on the other side.

In 2019, she got cervical spasms and also had very irregular heart rate. She also used to get panic attacks but they were controlled later. Sometimes while sitting her heart rate was 140bpm and sometimes 55bpm.

Her Mother died from familial leucodystrophy, she got herself tested but to get accurate results she needs a paternal sample along with hers which she is unable to get right now.

Now she doesn't know why she has so much pain while standing particularly in her abdomen to a point of almost fainting. It is bearable if she keeps moving and not standing at one place. Her fatigue is bad and even after no deficiencies, her extremities sometimes get easily numb.

What do you think is wrong with her?
