Anyone else feel like their 401k might not be set up right?

I’ve been getting more into investing over the past few years and learning a lot about portfolio management, especially with long term accounts like 401ks.

Something I’ve noticed is that a lot of people kind of set it and forget it,which is totally fine but sometimes those accounts aren’t really optimized based on things like fees, fund overlap, or risk level.

I’m curious how often do you guys actually check or adjust your 401k? Do you just leave it alone, or do you actively rebalance / change allocations over time?

Also, has anyone here ever found out their 401k was underperforming just because of how it was set up?
