Okay let’s establish a theory chain.

Nefertari D Lily is the ancestor of Vivi, their homeland has Egyptian themes which makes sense because nefartari sounds just like Nefertiti an ancient Egyptian queen.
Since we don’t know for sure what the D stands for let’s say for the sake of this conversation it stands for Dream.
If you look for Egyptian dream symbolism one of the first things you will find is the Blue Lotus , the Blue Lotus happens to be a Water Lily. Which is neat because Lily has blue hair, and we recently just saw what Zaza the Rain God looks like. It’s pretty crazy for this to just be a coincidence.
It’s also very coincidental the first time we see this mythical Zaza/Lily Figure it’s due to the dream/nightmare manifestations of those from the holy land.

Okay let’s keep going, I’ve seen a lot of theory’s on youtube talking about how since Blackbeard has a unique body seemingly capable of housing multiple devil fruits. Most of these theories include speculation that his parent(s) or ancestors would be capable of the same. Let’s assume Lily is someone with a unique body like Blackbeard, what other devil fruit would fit symbolism or wordplay associated with Lily?

Perennial is a type of flower characterized by the ability to stay alive “eternally” through its roots even when the rest of the plant is destroyed. Guess what kind of flower is a perennial, LILYS the Blue Lotus is a Water Lily, the Egyptian flower of dreams is an immortal plant!!
We know that one of the crimes Lily is guilty of has something to do with splitting up and spreading out the poneglyphs. Which theoretically could be done with the awakened ope ope no mi, perhaps in an act of defiance Lily created a huge room and teleported the poneglyphs to others on her side so they could be spread out from there. Imu deciding to kill her for this treason decides to force her to perform the perennial immortality surgery. The lily immortal surgery you could say.

Now this is gonna split into two different directions.
Direction A: the immortality surgery acts as a body swap and Imu is still in possession of Lily in some manner.

Direction B: the immortality surgery acts as a permanent area of effect that can either be placed on the volume of a person, or within the confines of a space. This would essentially be like having a “room” for permanent operation. The room is either stuck to a local or the being.

Direction A: Imu forces Lily to body swap, it kills Lily and her body is a lifeless corpse Imu puppets around in the holy land.
The figure we see fighting Luffy currently in elbaf is one of the 19 kings that willingly turned themselves over to imu essentially becoming one of 20 heads of the beast. Saint Imu Nerona refers to Nero, which many scholars belief is the antichrist referenced in revelations. (I don’t want to go there please let’s not derail the thread)

Direction B: Imu forces Lily to do the immortality surgery, thanks to him not understanding the mechanics fully she makes the flower room the immortal operation room(Lily, flower room, perennial) Instead of placing the rooms confines upon imus body she places the room on the room itself!!! The entire flower room is a perpetual youth healing safe zone where Imu is forced/ cursed to stay until her can get the next user to use the fruit on him how he wants.
This is why when Imu leaves the flower room he coughs up blood immediately, he just jumped off the operating table mid surgery and left the room. The gorosie are horrified because they know he is actively dying every second he fights in elbaf.

Perennials are flowers that stay alive via their root systems even when the rest of the plant withers away. Perpetually alive even if it’s not in the way you may think.
So now that we have established the immortality youth operation allows for a connection to Lilly let’s talk about the mechanics of the fruit and how this all comes together.

So we know that laws fruit allows him to create a “room” where he can operate within it. This immortality surgery takes the users life to execute.
Let’s assume instead of just making someone immortal the move actually acts as a field of perpetual healing, like a safe zone. If you are within the confines of the operation room you will be kept alive and healed.
If we take the stance that this operation was done on or for Imu it could also be safe to assume it wasn’t by choice.
Law today doesn’t seem eager to give up his life, we could imagine if Lily had the fruit this could be a similar case.
So assuming someone was forced to perform the surgery for Imu we can assume they might have been domi reversi’d.

We just saw Gunko in a flashback while domi reversi’d spare brooks life which would be safe to say was her excepting her own will to change her actions and disobey.
I’m theorizing that Lily was forced to perform the immortality surgery on Imu in the same room that is today the flower room. While being forced to do so she set the confines of the surgery on the room itself instead of on Imu.

This final act of defiance has now essentially trapped Imu in the flower room and away from the empty throne which he wishes to sit upon full time without requiring a return to the flower room. We see him take damage upon leaving the room, almost like he moved during surgery :)
