If you live in Eastern Ontario and do not have a family doctor or nurse practitioner, you can still access provincial breast, cervical, colorectal, and lung cancer screening programs.

The Champlain Screening Outreach Nurse Practitioner can help determine whether you are due for cancer screening and connect you with the screening services available to you.

How to Book with CSO Nurse Practitioner

You can self-refer by:

Your first appointment will be conducted by phone. If an in-person visit is needed, appointments are available at various locations throughout the Champlain region.

Ontario Cancer Screening Eligibility

  • 🩺 Cervical Cancer: HPV screening every 5 years for people aged 25–69 with a cervix.
  • 🎗️ Breast Cancer: Mammograms for people aged 40–74. Earlier screening may be available based on family history.
  • 🚭 Lung Cancer: Current or former smokers aged 55–80 may be eligible.
  • 💙 Colorectal Cancer: FIT (stool) testing every 2 years for people aged 50–74. Some individuals may qualify for a screening colonoscopy based on family history.
