Hey everyone! My name is RaeRae and I am a 38-year-old non-binary trans masc. I am currently waiting for my top surgery. I found an amazing surgeon, Dr. Cornelio, in Brazil, to perform my mastectomy. She specializes in transmasculine mastectomies and she is so great! I had to push back the date for my surgery because it is taking me longer to raise the funds than I thought. A little backstory, first: My wife and I moved to Uruguay in 2017. We have socialized medicine here and the government pays for gender affirming care. There was a trans law passed here in 2018 because of the history of transphobia, especially under the former Uruguayan dictatorship. This law is part of the government’s current attempt to right past wrongs and give trans people equal rights under the law. They are supposed to give us all access to easy and quick GAC and GAS through the government’s sexual diversity clinic. However, there are only a handful of surgeons who are trained in specific procedures, and the one hospital that does GAS over the past few years hasn’t been blocking off time in the surgical schedule for GAS. There is now an average of only 2 being performed per month. There are over 100 trans folks on the list, and that means it would be a wait of over 4 years for me to be able to receive top surgery through the public healthcare system. I am on the list for a hysterectomy, but I am fine waiting for a few years for that procedure.  I cannot wait much longer for top surgery. Three years ago, I was diagnosed with a rare form of lung cancer and developed a bilateral pleural effusion, which is when the area around your lungs fills with fluid. I survived, but while healing after the surgery, my top dysphoria got really bad. I was constantly dissociating from both the physical pain and the dysphoria. All I could think about was that I survived cancer to potentially spend the rest of my life growing old in a body that is not totally mine. For over a year, I had pretty severe pain around and throughout my chest, and I could not wear anything tight, let alone a sports bra or binder. I was worried no surgeon would be willing to work with me, so I waited a few years in a lot of emotional pain and dealing with medical PTSD. I cannot wait much longer. My dysphoria and depression are really bad, and I am disappointed I had to postpone my surgery for lack of funds. My fundraiser is not doing very well, and I am asking you, my trans sibs, for help. Need suggestions about how to talk to plastic surgeon about the reduction procedure not just being for pain, but also GAC

I moved to from the US to Uruguay with my wife years ago. We have socialized medicine, so the government here pays for both reductions for physical pain, and mastectomies for GAC. (We don't need to see psychiatrists here for GAC for hormones or surgeries.) I am non-binary trans masc, and I have my reduction consult this week. Since I am looking to get a radical reduction top surgery, and not a double mastectomy, I didn't tell the clinic doctor the trans part. I just told her I needed the surgery for pain, and she wrote the prescription. I reached out to the hospital that she referred me to, and I never heard back. I actually asked the clinic for help again, because it was taking so long to hear back. Another doctor at the clinic wrote me another prescription AND reached out to the hospital for me. And I finally got my consult scheduled! One concern is that I am afraid they will only let me go down a few sizes if it is approved solely because of the physical pain aspect. I know in the US, surgeons frequently aren't willing to go down more than a few sizes, and I am a 40G or something close to that. It feels like if they say they will only remove so much for pain, then my only other choice is to postpone the surgery and to wait YEARS on the GAS list, which as I mentioned before, really only exists in its current form for mastectomies. Or raise a shitload of money to get it done privately.

The GAS process is hard to navigate for non-binary people who want radical reduction top surgeries, because there are so few people (none I know of) who want this procedure in Uruguay. (I know this is a thing everywhere, but we only have a population of 3.4 million people, and fewer total trans people.) There are binary and non-binary trans people who get GAS here, and surgeons who specialize in them, but I have not found any other people or surgeons who have knowledge/experience specifically in radical reduction/non-binary/androgynous top surgeries. The ones who do GAS specialize in both mastectomies with and without chest masculinization, but I don't think the surgeons who do these GAS know much more about radical reductions than the average plastic surgeon who does reductions for cis women do. *sigh* I am just wondering how to approach the subject, because I NEED the surgery asap. I have been waiting years. How do I bring up that I need them SMALL and not just a few sizes smaller?

(I am Mad and multiply-neurodivergent and this is LITERALLY making me incredibly emotionally and physically ill.)
