Need Advice

So, for context, I graduated from my undergrad with a 2.32 GPA (Bad yeah). Since then Ive done about 80-90 credits of DIY at a local 4 year CUNY with all A's. Say I raise the total cumulative GPA to a 3.0 at least, would I have to do an SMP to guarantee an acceptance, or can I simply just apply straight away. Will be taking the MCAT next year, but have been scoring around 512-515 on the practice Kaplan exams (AAMC will be done closer to exam date). I currently have about 6000 PCE as a MA/Medical Coordinator, and gathering more research until next May (Maybe around 500 hours?). I just need some inspiration because this road has been long and hard, and I have moments of doubt. I really want to become a physician (IDC about DO/MD, either I would happily accept). I just want to know that this road will hopefully lead somewhere. Ive changed much from the student I used to be, and working at a cancer center has really reaffirmed my love for medicine.

Please just be blunt and honest with me, I really need it more than ever. I'm nearly at a 3.0 cumulative GPA, I'm just taking a few more upper division BIO/CHEM courses to pad my transcript. Currently enrolled in Inorganic Chem, Genetics and Nutrition for this Fall while working full time.

Give it to me straight, be honest

So, to start off, my undergraduate GPA is a joke. I graduated from Stony Brook University with a 2.3 GPA (terrible I know). Since then, I have worked as a Medical Lab Assistant at Memorial Sloan Kettering for about 1.4 years. Then another 1.5 years at NYU Langone as a Medical Assistant, and now I'm currently working as a Care Coordinator at MSK again in the Chemotherapy Unit. I am currently doing an Associates at CUNY City Tech and some non-degree courses from Hunter College and have about a 3.871 GPA with about 90 credits. I am planning to take the MCAT around next April (AAMC Tests Ranging between 516-520)

My letters of rec are amazing from my oncologists, as well as my shadowing. I do have a publication with an oncologist from MSK and plan to do volunteering when my pre-reqs slow down.

My question is: What is the best course of action to take? Do I do another Masters after this associates? Or just do all of the pre-reqs at City Tech? Do I go for MD or DO? Or just say screw it and hope PA is a last resort.

It's just been very humbling to see how my low GPA really has opened my eyes to how little I can do with it. I want to be a in the medical field, truthfully, I do. It just sucks that I didn't take it seriously when it really mattered. So, to those who haven't fucked up as hard as me, please give it to me straight.

Here are all my post bacc credits since graduating and my cGPA is nearly averaged at a 3.0.

Again, I just want to get into a medical school, I dont care about t20 or t10 schools, I just want to be a physician.
