Major scalp issues for ~5 years. Red, painful, itchy circular patches that have caused hair loss

Hi, I’m a 30 year old female, ~145 lbs, not a smoker.

I’ve been having issues with dandruff since I was 19. I used dandruff shampoos religiously until I was 25, and that seemed to control it.

However, in the last 5 years, I’ve developed these circular, red, painful, spots on my scalp that seem to be completely bald, as well as shedding *excessive* amounts of hair. I’ve had this issue for about 5 years, but it’s become significantly worse in the last 2 years.

I have tons of pictures of the progression from the last 5 years but it seems like I’m limited with the amount of pictures.

I’ve tried every dandruff and hair regrowth shampoo/conditioner/serum OTC and even a few prescription. Been prescribed antibiotics and antifungals at least two times each, and it only seems to be getting worse.

My only major health concerns are anxiety, high BP (genetic), ADHD, and Ulcerative Colitis and a history of two MRSA removal surgeries (one on nose and one on elbow). I was taking Pentasa to control the UC but I had lost my health insurance for about a year and only got it back the 1st of June.

I currently take Cymbalta, Losartan, and adderall. Again, the pentasa had to be stopped because it was almost $700 a month 💀

So obviously my plan is to find a primary care and hopefully get referred to a dermatologist (again)? Is there something else I should be looking for or concerned about?

I’m just at my wits end. It’s so embarrassing to be losing your hair in weird patterns at only 30. Most of my coworkers get their hair dyed and I’m too embarrassed to even go to go get a simple haircut. I was even ‘warned’ about work about professionalism for not ‘brushing my hair’ when in reality it’s 1. Broken and fallen in weird ways and 2. I sometimes part it weirdly to hide the spots.

I’m just really lost and embarrassed and was wondering if anyone had some idea of what could be causing this.
