Enrolling in optum pay as a sole proprietor is making me want to lose my mind. I've been declined 3x now. I just called, but the rep didn't add any insight into what I'm doing wrong. My voided check is somehow an issue as well as my W-9 and CP-575 (which I had to upload on this last attempt). I don't know why. My EIN and voided check still have my home address associated with it; is that part of the issue? Do I put the account under my personal name, since it says avoid DBA if possible? If anyone has a step-by-step guide on how to get this done, I'd very much appreciate it!
I received my contract from Optum but it lists an old practice address that I'm no longer affiliated with. Should I go ahead and sign the contract or reach out to Optum? I also can't find a way to reach out to the "Network Management Team" to correct it.
My period started on day 19 and continued for another few days, while I was cycling my progesterone. It wasn’t full blown but light bleeding with cramping. I’m now on day 25 and I don’t think a full period is coming. Now I’m so confused about when to start my next cycle. Do I just count day 19 as day 1 even though I continued the progesterone? I’m probably overthinking this 😂
