[looking for game] Top-down pixel art Lovecraftian horror metroidvania where the protagonist wakes up chained to a sacrificial altar

Hi there. I've been thinking about this game for years now but haven't found it yet...The main character wakes up chained to a sacrificial altar, about to be sacrificed by a cultist. A flying skull appears, helps the protagonist kill the cultist, and frees him from his chains. After that, the player fights through the level, killing the remaining cultists while progressing through the story. The dialogue with npcs often had a slightly sarcastic tone. That's pretty much all I remember
