Small update/changes

I was trying to avoid posting lol but I’m looking for some slight insight if possible. It exhausting to run down the timeline, but since my emg, almost 1 month ago, I’ve noticed a slight change. My right calf has become weaker, that I’ve noticed. I can no longer lift off my heel to a self-weight bearing toe stand. I can try but my foot instinctively leans to my smaller toes causing a loss of balance and just not able to lift with strength. I noticed the change four days ago. Driving has become a little difficult the past few days due to that instability. My calf will begin to cramp/ache while constant movement break to gas pedal. I’ve had mris, X-rays, emg. Waiting for nerve ultrasound next week, Monday. I’m curious now if they could check behind my knee or further up my leg also to spot what could be causing this sudden change. Has anyone experienced this weird change? My emg findings are in a previous post but for summary: acute-to-subacute, non-localizable, right common peroneal neuropathy, a conclusion based on the small right superficial peroneal SNAP, relatively small right peroneal CMAPs, and EMG findings of Fibs/PSWs with minimal/no chronic neurogenic changes in the right tibialis anterior and peroneus longus. There is no focal slowing across the fibular head. Thank you in advance for any insight or similar experiences.

Emg/ncs results

Hello everyone! I post last week about my right foot drop and weakness. Here is my EMG report. Any insight would be helpful!!

SUMMARY OF FINDINGS:

  1. Right peroneal motor NCS, recorded from the EDB and TA, were within normal limits in terms of absolute values, but the distal CMAPs were relatively smaller on the right compared to the left. Right tibial motor NCS was normal.
  2. Right superficial peroneal antidromic sensory NCS (performed in duplicate) showed a small SNAP amplitude and a normal conduction velocity. Left superficial peroneal antidromic sensory NCS was normal. Bilateral sural antidromic sensory NCSs were normal without any notable side-to-side difference.
  3. Bilateral tibial H-reflex studies showed normal minimal latencies without any notable side-to-side difference.
  4. Needle EMG of the right tibialis anterior showed 1-2+ Fibs/PSWs, MUPs of increased duration and reduced recruitment during periods of adequate activation. EMG of the right peroneus longus showed 1-2+ Fibs/PSWs and reduced activation and was otherwise normal. EMG of the right gastrocnemius (medial head) showed reduced activation and was otherwise normal. EMG of the right biceps femoris (short head) and tensor fasciae latae was normal.

Temperature was maintained above 30°C in the foot for all NCSs.

CONCLUSION/INTERPRETATION:

This study provides electrodiagnostic evidence of an acute-to-subacute, non-localizable, right common peroneal neuropathy, a conclusion based on the small right superficial peroneal SNAP, relatively small right peroneal CMAPs, and EMG findings of Fibs/PSWs with minimal/no chronic neurogenic changes in the right tibialis anterior and peroneus longus. There is no focal slowing across the fibular head. A neuromuscular ultrasound could provide additional information, if clinically indicated.

Symptoms update

I have a post on here about a week ago explaining my symptoms. I’m leaning towards Ataxia with what I know so far:

I still have “drop foot” in my right foot. Unable to heel walk on right but able to on left. Can stand on toes but can not take more than 5 steps without right right giving out or “rolling”
I have no pain (except for leg exhaustion/ache by end of day) my lower back and pelvis has been taking the brute of my abnormal gait I’ve been walking with. Still waiting for AFO fitting in October.

I go for my EMG/NCS Wednesday morning Sept. 9th. I will come back with any updates, to see if there could be anymore insights from anyone else with similar experiences.

Possibly Limb onset (mnd) or MS??

33yr old female, overall past great health 5’3 around 135 lbs. I’m am currently getting medical help also.
Wanting to see if anyone on here could help understand what could be the issue..
JULY 21 this year, I went to a concert and noticed my right foot was slapping the ground. Went the whole day not really caring or thinking anything of it. Two days later woke up with a completely weak right foot. Had a virtual appointment and was suspected foot drop. Went to urgent care, because of tremors in that leg, weakness and foot drop, doctor suspected stroke. Went straight to ER. Had CT of head, xray of back. Everything normal. Sent home with foot drop. Had PCP appointment a week later. Explained all symptoms and timeline. She referred me to neurologist and physical therapy. Saw neurologist a few weeks later. She did a physical examination, had me walk, (I have developed a noticeable gait abnormality) so she scheduled a BRAIN MRI WO CONTRAST, LUMBAR SPINE WWO CONTRAST, EMG/NCS, had LUMBAR XRAY SAME DAY IN CLINIC, BLOOD WORK (I think 8 tubes) SAME DAY, and SCHEDULED PRO BONO PHYSICAL THERAPY (Medicaid not covering regular PT)
So Far, I have had everything done except the EMG/NCS. Everything else was clear and enough to rule out it is not causing my symptoms. Had Brain MRI results within 48 hrs, clear results, no concerns. STILL WAITING ON LUMBAR MRI RESULTS: 6 days waiting so far. I am still experiencing foot drop in right foot. I cannot walk on my heels without my right foot slamming down. I have muscle twitches in both legs, that is, the calves and thighs. The twitches really prefer my left upper thigh. The twitches feel like a fat worm rolling around or moving in same muscle spot. No pain, just annoying and distracting. Since having these symptoms, I’ve noticed my right leg is pretty much loosing its strength. I have had my first appointment with pro-bono therapy. They stated they can’t help much because of it being a central nervous issue to them. They can help maintain the strength I have. I will be fitted for an AFO beginning October. The therapist were open enough to let me voice my thoughts. I stated how I think this could be MS and they agreed it’s a possibility. I was sent home with a rubber band as a make-shift AFO in the meantime. I use it often. MY EMG/NCS is next week. Hoping to get my lumbar spine MRI before then to have some kind of idea or answer. My neurologist has been very vague with me and so has my PCP. I feel discouraged but I also understand they are waiting to eliminate any possible cause. PLEASE! If anyone can help give some insight I would be very thankful!!! I am also open to explaining more specific symptoms, timeline, or medical background. Thank you!!

EDIT:
Just received result for LIMBAR MRI WWO CONTRAST.
Doctors message:
Your MRI of your lower spine did not show any abnormality that would cause your right leg weakness. The MRI was one of two tests we planned to look into possible causes. Your EMG scheduled for 9/9 will be the next step in looking for a cause.
They also noted in results
There is a small central and left paracentral disc protrusion abutting the left lateral recess and exiting S1 nerve root. There is mild bilateral L4-5 and L5-S1 facet arthropathy. There is no neuroforaminal stenosis. There is no spinal canal stenosis.
All my other lumbar finding were normal…
