Hello. I live well into Long Island and had no way of getting to work the two days during the LIRR strike. My payroll secretary said those days would be coded as personal data and taken out of my CAR. However form OP-201 allows being excused for up to two consecutive days with superintendent approval. Thoughts? Thanks.
Good Afternoon, I wish to apply for a 6 month restoration of health sabbatical for weight loss. I am morbidly obese with herniated disks in my back and no cartridge left in my knees. As a result I require use of aqua therapy and the community swimming pool and physical therapy all of which are closed by the time I get back home from school. Is this something that would be plausible with medical documentation? Thanks for your input.
