Looking for advice for my little brother starting 5th grade with a urostomy

Hi everybody!
I’m looking for some advice for my little brother, Ben, who has had a urostomy since he was around 7 years old. He was born with bladder exstrophy, and getting to the point of having his urostomy was a long process for our family. He dealt with recurrent bladder infections that resulted in febrile seizures, life flights to Mayo, provider denials, and a lot of uncertainty while my mom was trying to get him the care he needed. Eventually, after a lot of work, he was able to see a wonderful pediatric urologist and a loving care team at Mayo who helped us figure out the best path forward, and he ultimately received his urostomy. He is now getting ready to start 5th grade next week with his twin sister. Ben has always been pretty accepting of his bag, and I think he has done really well with it, but as he has gotten older, he has started to notice some of the social differences. One thing that has been bothering him is feeling like other kids don’t invite him to sleepovers at their houses the way they do his sister. I think he is becoming more aware that his bag is different, and I want to help him feel comfortable and confident as he gets older.

One thing we have been struggling with lately is the little belt he currently uses. It has been rubbing against his stomach, and we have tried Band-Aids and other things to protect the area, but he is a very active kid and they don’t always stay in place. I attached a picture of the belt he currently uses for reference. I’m also wondering about the more supportive ostomy belts or wraps that actually cradle the pouch and keep it closer to his body instead of letting it hang freely and pull downward when there is urine in it. I’ve seen some online, but most seem to be made for adults, so I’m not sure what would work well for a 10-year-old. I’d love recommendations for something that would give him support without restricting his movement while he’s at school, playing, or running around. He also sometimes forgets to empty his bag as often as he should. We do our best to remind him and have tried timers, but there are obviously times when we can’t. If anyone has tips that helped a child become more independent with that, I’d love to hear them. We’re also looking for recommendations for bag covers that work well for kids. He wears shirts over it, but I think having a little extra privacy could help him feel more comfortable as he gets older.

For some context, I’m his older sister. I’m 25, in college, and I’ve had to take on a pretty big caregiver role for my siblings growing up. I don’t make a ton of money, so I’m hoping to find options that aren’t going to cost an arm and a leg. I currently work in healthcare, so I understand that medical supplies can be expensive and that sometimes the more expensive option is worth it. We’re very fortunate that my mom has good insurance and that his necessary supplies are covered, and we’re not looking for someone to provide those for us. I’m really just trying to find some of the extras that could make things easier for him, whether that’s an affordable product, something worth spending a little more on, or even a simple trick that another family has found helpful. I’ve spent a lot of time searching online and I’ve used Reddit for advice on other things in the past, so I figured this would be a good place to ask people who actually have experience with ostomies. I don’t want to accidentally make things harder for him or turn something that he has already learned to live with into another challenge.

More than anything, I want Ben to be able to start 5th grade and just be a kid. He’s outgoing, lovey, inquisitive, funny, and incredibly resilient. He loves being active and keeping up with everyone, and I want him to be able to stay that way, go to sleepovers, and do the things he enjoys without feeling like his urostomy has to limit him. If you have a child with a urostomy, grew up with one yourself, or have experience with pediatric ostomy supplies, I’d really appreciate any advice, recommendations, or things you wish you had known at his age. I’m really just trying to do right by him and make sure this is something that feels accommodating and supportive rather than something that makes his life harder. 💕

Is anyone else’s hospital pharmacy struggling this much??

I don’t want to say the name of the hospital out of respect, but I work in the pharmacy at a Level I trauma center and I’m wondering if anyone else’s hospital is dealing with the same kind of issues. Our hospital has around 380 beds and we’re pretty much always full. We’ve had to open up surgical admission units just to make room for patients, and there are admitted patients staying in ED rooms because there aren’t enough inpatient beds. The ED is constantly slammed. Even with that, evenings in the pharmacy are expected to get by with one peds pharmacist, one ICU pharmacist, one ED pharmacist, one pharmacist covering everything else, and about 4-5 techs/interns. A majority of our evening pharmacists are also remote now, which has made things even harder because so much still depends on having people physically in the pharmacy. To make matters worse, most of our evening pharmacists can’t transfer off evenings because of seniority, so they’re essentially stuck working one of the hardest shifts.

A few months ago we were told pharmacy spends “too much money,” so our evening pharmacist incentive was taken away. Techs and interns also don’t get incentive anymore for picking up extra shifts. The only differential we have is nights and weekends starting at 9 PM, so most people working 1:00-11:30 don’t really see much of it. Meanwhile, other departments are getting incentive pay, double time, or bonuses to help with staffing. Our evening pharmacists have basically taken a $10,000-$20,000 pay cut to keep working the same shift that nobody wants, and we’re already one of the lowest-paying hospitals in the area. We’re constantly being told there isn’t any money for pharmacy, yet we’ve continued to see management positions being promoted or added. From where many of us are standing, it’s hard to understand those decisions when we’re being told we can’t afford to retain staff or offer incentives to the people working some of the toughest shifts. It feels like we have too much management while the people actually working on the front lines are expected to keep doing more with less. We also keep getting new policies, new workflows, and more meetings, but a lot of us feel like those changes make it easier for people who don’t pull their own weight instead of holding them accountable. The people who consistently work hard just end up doing more.

The worst part is what it’s done to the department. Most of us really do love what we do. We care about our patients, and we care about doing our jobs well. But everyone is burned out, people are frustrated, and it’s created a pretty toxic work environment. A lot of the tension ends up being between pharmacists, techs, and interns when the real problems are way bigger than any of us can fix. It feels like we’re all just trying to survive the shift while being asked to do more with less. Instead of feeling appreciated for keeping things running, it honestly feels like we’re being punished for it. I’m really just wondering if this is becoming normal at other hospitals or if it’s just us.

Need Help With Apartment Complex Floor Plan

Hey everyone. I’m looking for help with the first floor floor plan of an apartment complex I’m building in The Sims. I really love the exterior shape and overall design of the building so I want to keep the outside structure the same, but I’m struggling to make the inside layout flow realistically.

Right now I’m trying to fit:
•2 studio apartments or small 1 bedrooms
•lobby/seating area
•reception/front desk
•mail room
•gym
•public bathroom

Any interior walls can be modified, rearranged, or completely removed if needed. The only thing that needs to stay is the elevator area in the middle/back section since I want that to remain easily accessible from the main entrance/common areas.
I’d really appreciate any floor plan ideas, sketches, or suggestions for how to make the layout feel more natural and functional. TIA!!

Help w/ apartment complex floor plan

Hi Simmers! I’m working on an apartment complex build in TS4 and could really use help figuring out the first-floor layout. I really want to keep the outside shape mostly the same, but the inside layout just isn’t flowing right and I keep overthinking it!

I’m trying to fit:
•2 studio apartments
•lobby/seating area
•reception/front desk
•mail room
•gym
•public bathroom

The middle/back area already has an elevator that needs to stay easy to access without walking through apartments or shared spaces. Later on I also want to add a garage entrance on the front left that leads to underground parking, plus a patio/pool area in the back left. Those can come later, but I’m open to ideas for how they could connect now.

Feel free to totally rearrange or remove interior walls if needed! I mainly just want to keep the exterior shell. Also just to clarify, the shell isn’t my original work, this is only for personal gameplay/build practice and I’m just customizing the interior for my save. All credit goes to the original creator!

I’d really appreciate any layout ideas, floor plan sketches, or examples of apartment/common-area flow. TIA!!
