Cat pooping all over apartment-appears to be in pain ☹️

My cat started pooping outside the litter box and occasionally vomiting almost 2 years ago. His bloodwork and fecal tests have all been normal. Vet recommended prescription “GI biome” diet. He got a lot better and I thought the issue was fixed. He would still occasionally vomit and pool outside the box but very rarely.

Since April this year he has been having more episodes. I took him to the animal hospital early April because he had severe vomiting and diarrhea with lethargy-they gave him vitamin B12 and fluids and he seemed better. I also treated him for fleas since a vet saw flea residue on him and thought he may have flea allergy.

He has pooped outside the litter box everyday now for almost 2 weeks. He seems to be in pain and he will start going in the box then jump out and run around my apartment while pooping. It’s awful.

He has a vet appointment on Friday. They are recommending an ultrasound. I am wondering if it could be IBD, lymphoma, or some food allergy? He seems like he doesn’t digest food well and his poop has a lot of mucus.

I hate seeing him in discomfort. It’s also very frustrating to have to clean cat poop all over my apartment everyday 😢

If anyone has some advice let me know. He gets probiotic with meals. I feed wet and dry prescription food.

I posted a video of his behavior usually before this happens. He will run fast while pooping so that’s hard to catch on video. His poop usually is separate hard lumps with a lot of mucus or runny.

Thanks

EDIT: I have taken him to the vet many times over the last 2 years, including 3 visits in the last 2 months. He has had bloodwork done, X-rays, and fecal tests. The vet is not sure what is going on so they recently recommended an ultrasound and I have an appointment Friday. I am reaching out here to see if anyone has had similar issues with their cat or any advice. After he poops outside the box he appears asymptomatic. He is still eating and drinking normally at this time. Since this has been a chronic ongoing issue I don’t feel there is a reason to take him to an animal hospital as it’s not an emergency right now.
Simply telling me to take him to the vet is not helpful at this time.

Changing gloves before removing PIV

I’ve been a RN for 3 years and I’ve had two patients family members, that I can remember, ask me to put on a new pair of gloves before taking out the patient PIV. I don’t recall getting trained to put on new pair of gloves for removing patient PIV. I was charting on the computer and got the patients vitals prior. I always change gloves after touching soiled items. Just curious is this common practice? I felt bad and am not sure if I’m missing something
