I got by a YJ on Friday and tried some otc stuff like cortisone and ibuprofen but went to an urgent care yesterday and she prescribed me a topical antibiotic as well as an oral one. I drew a line around it to watch if it spreads and as of yesterday it hasnt except one part was closer to the line but not over. Swelling and pain are down. I will provide photos, its on the back of my calf

How do get restrictions without FMLA? Does the person who writes it for me have to have an MD or can it just be my therapist through whom my FMLA was through?
Also, is there a specific kind of wording that is required?

Im looking at my case files, it says medical evidence to support disability is required for compensation. My case is approved but im not permanently disabled, i missed a lot of work but Ive been back to work. My leave was intermittent, whenever i needed to which was sometimes weeks or months.
Im awaiting my steward to set up another meeting after getting what he needs from management for us to start filling out CA7s so I know i can talk to him but if i need documentation, i want to get ahead of it.
My question is do i need additional evidence from a dr for my CA7? My case has already been approved, its been designated as temporary not permanent as of right now. I was thinking since my case is approved, i shouldnt have any issues with a CA7 but i am unfamiliar with the process.

I made about 1350 doordashing this year doordashing. Is that something to report when filling out a CA7?

I contacted a former office today to get my records from them and they said they would only send me redacted files. But to a lawyer, owcp, a business etc they would send them unredacted. I want to see my records unredacted before I send them in somewhere. Im in Ohio, is this legal?
Also this was the absolute worst therapist ive ever had, he was constantly nodding off in the middle of sentences, would forget where he was, who I was, would ask me questions like I was a different patient, etc. all around awful behavior, It made me think he was on heroin truly, he would just fall asleep and trail off and slump.
That makes me suspicious of them now, any help would be greatly appreciated
