Chronic neck/trap pain with one-sided headaches, sitting and driving becoming unbearable

I’m trying here because I’ve been struggling for several years with neck/traps/upper back pain and I’m wondering if anyone has had something similar

It started around 3 years ago, at first it was mostly triggered by sport, especially running, after a run it was basically guaranteed headache. I would feel a tension that often started from the base of the skull, kind of around the occipital nerve area, then it would turn into a migraine/headache on one side. Never both sides at the same time, more often on the right. Duration around 24h, sometimes strong pain with nausea/feeling like vomiting, but relatively relieved by stretching/sport, nothing much to do except go to bed basically.

Over time it got worse or at least spread, now it’s not only sport anymore. Sitting becomes unbearable very quickly, especially in front of a screen, I feel like no matter the setup, it pulls in the traps/cervicals, then if I push through too long it can turn into a flare with pain at the base of the skull and migraine. Even driving or being a passenger during a flare can hurt.

No matter how I adjust the seat it becomes unbearable very quickly, if I lean the backrest backwards, it pulls in the upper back/cervicals, if I overcompensate and try to sit straight, I get extreme tension in the upper chest/neck and traps.

I’ve been seeing a physio for about 1 year. We mainly do McKenzie and light strengthening, but for now I don’t feel like I’ve found a clear solution or something that really fixes the problem

I also do bodyweight strength training pretty regularly (3x per week with stretching, chin tuck / back extensions etc) so I’m not totally sedentary, but I also spend a lot of time in front of a screen. I’m struggling to know if it’s more of a postural issue, muscular, cervical, occipital nerve, cervicogenic headache, tension headache, or a mix of all that.

What I mostly feel:
pain in traps / upper back / cervical area
often starts at the base of the skull around the occipital nerve area
headache or migraine when it gets pushed too far
one side at a time most of the time
sitting in front of a screen is very difficult
standing is much more tolerable
driving is difficult during flares

Douleurs cervicales/trapèzes + céphalées depuis 3 ans, assis et conduite devenus compliqués

antécédant : cyphose assez élevé ( bosse haut du dos ) et légère scoliose

Je tente ici parce que je galère un peu depuis plusieurs années avec des douleurs cou trapèzes haut du dos et je me demande si certains ont eu un truc similaire

Ça a commencé il y a environ 3 ans, au début c’était surtout déclenché par le sport, la course a pied particulièrement, retour de course c'était céphalée garantie. je sentais une tension qui partait souvent de la base du crâne, un peu zone nerf d’Arnold, puis ça remontait en migraine/céphalée d’un côté. Jamais les deux côtés en même temps, plus souvent à droite. durée environ 24h, douleur parfois forte avec envie de vomir, mais relativement alégée par des étirements/sports, rien a faire a part aller se coucher en somme.

Avec le temps ça s’est aggravé ou au moins élargi, maintenant ce n’est plus seulement le sport. La position assise devient vite insupportable, surtout devant un écran, j’ai l’impression que peu importe le réglage, ça tire dans les trapèzes/cervicales, puis si je force trop longtemps ça peut partir en crise avec douleur à la base du crâne et migraine. Même conduire ou être passager en période de crise peut me faire mal.
Peut importe le reglage de l'assise c'est insupportable très rapidement, si je met le dossier en arrière, ça tir dans le haut du dos/cervicale, si je compense trop et que je tente de me tenir droit, tension extreme haut du torse/cou et trapèze.

Je vois un kiné depuis environ 1 an. On fait notamment du McKenzie et du renforcement léger, mais pour l’instant je n’ai pas l’impression d’avoir trouvé une solution claire ou un truc qui règle vraiment le problème

Je fais aussi de la musculation au poids du corps assez régulièrement (3 x par semaine avec étirement( chin tuk / extension buste etc ) donc je ne suis pas totalement sédentaire, mais je passe beaucoup de temps devant l’écran. J’ai du mal à savoir si c’est plutôt un problème postural, musculaire, cervical, nerf d’Arnold, céphalée cervicogène, tensionnelle, ou un mélange de tout ça.

Ce que je ressens surtout :

départ souvent base du crâne côté nerf d’Arnold
céphalée ou migraine quand ça force trop
un seul côté à la fois la plupart du temps
assis devant écran très difficile
debout beaucoup plus tolérable
conduite difficile en crise
