My boss is more hands off regarding how we spend our time at work, so long as the work is done correctly on time.
I tend to procrastinate, and waste time, then rush it through at the end. If I were to work consistently, the work would get done without issue.
Any advice on how to hold myself accountable?

My manager told me we would have a meeting on Monday to discuss how to improve my work.
There would be weekly video calls to go over my progress, and a plan to get me back to fully meeting expectations.
Would this be the equivalent of a pip, where it's improve or be fired, or are we not there yet?
I've had some poor discussions with my manager on my work, and based on my mediocre performance and my meetings, I think I will be placed on a PiP.
