Reintroducing Soy

Baby girl just turned 7 months and we wanted to start working on the soya ladder with her. She never had horrible reactions, just added fussiness, mucus and minor blood in the stool.

With starting solids, her poops no longer have mucus and I definitely wouldn't be able to see trace amounts of blood.

I guess unless she's incredibly fussy, I can assume she isn't reacting?

We gave her fermented soybeans (natto) today for lunch and she devoured them and has been her normal self all day. Would you consider that her clearing that tier of the ladder and I can reintroduce fermented soy to my diet? Or would you give fermented soy a few more days in a row before calling it clear?

Testing Other Grains

We believe baby girl has FPIES to oats. Her older five year old brother has confirmed FPIES with triggers of egg and cashews.

For parents whose baby has FPIES to a grain, if they reacted to another grain (rice, quinoa, wheat, etc) did they react the first time they had it?

Just trying to sort out what foods we can say are "clear" though we know it can take multiple exposures before a reaction occurs. Just wondering since they might be linked would she react to the first exposure of the new grain.

Am I Clear To Eat Oats?

Please forgive me, this is probably insanely obvious and a dumb question but my overly worried and stressed child with MSPI brain is overthinking this.

We THOUGHT oats was a trigger, I had oats and she was fussy and had blood in her stool the next day. But we recently found out beef was the missing trigger. I checked my food diary at the time and confirmed I had beef the same day as the oats.

We just started on solids and tried baby oatmeal first and she's had it three days in a row. She had a "normal" (I'm not even sure what normal looks like at this point anymore) poop today and hasn't been extra fussy or anything.

Has she proved oats isn't a trigger for her?

Is baby oatmeal the same as normal oatmeal so I can have oats again? I'm pretty sure they are but then you'd assume soybean oil and soy lecithin would trigger a baby sensitive to soy but for most it doesn't and some it does and it doesn't even have to be listed as a soy allergen so here we are with me making sure baby oatmeal means I'm safe to eat oats again! 😂

Ate Oats Yesterday and Blood Reappeared

The pediatric GI I saw was very adamant if the blood specks in the diaper didn't stop soon, to stop breastfeeding and switch fully to formula. This was only a week after eliminating only dairy. I cancelled any future or follow up appointments with him.

I was very adamant about breastfeeding so I also eliminated soy, egg and oats from my diet in one go. Lo and behold, baby girl stopped having blood specks within a week. So of course, that also means I don't fully know what the actual trigger(s) are. Yesterday I ate two oat granola bars and overnight baby girl was very fussy, refusing to go down in her bassinet. This morning she pooped twice and the second time there was blood again.

I assume this swift reappearance of blood confirms oats as a trigger? My husband was curious if oats could be the only trigger or is it assumed if they can't tolerate oats that dairy and soy are also triggers?

So Much Conflicting Advice

We just did basic bloodwork on baby girl and she has low iron saturation. Her platelets are also elevated, both can be hallmarks for chronic inflammation.

I keep reading that as long as they're growing, aren't extra fussy, that even blood specks in the poop is fine and to continue breastfeeding.

I've cut dairy and soy (some food has soybean oil in it which I read most babies can tolerate) from my diet for almost two weeks and yesterday baby girl had a green poop when it's normally just yellow. She has lessoning of the blood specks but a swap test at the pediatrician showed she still has trace amounts of blood in her poop.

Basically just rambling and spiraling on 4 hours of sleep after reading the blood results at 4am (terrible idea).

I plan to make an appointment with a pediatric GI in the morning. I had seen one previously and he basically said give it a week and if she's not back to normal to formula feed. But again, I've read as long as they're thriving to just keep BFing and they usually outgrow it. Some people do elimination diets for months all while baby still has bloody stools. But seeing all these hallmarks for chronic inflammation for her is terrifying me. But I also read formula is sometimes rejected by them too and if my supply is tanked by then there's nothing I can do, my statsh clearly wouldn't be safe for her. I had to exclusively pump for my first baby and it was horrible.

Just a very lost, confused and scared mama right now.

MSPI Journey Thus Far

At onset of symptoms, 3.5 months old EBF

4/23 Green stool with blood specks

4/24 - 4/26 On and off blood specks with normal colored stool

4/27 Cut all forms of dairy

4/28- 5/4 Continued on and off specks of blood, normal colored stool

5/5 Cut soy, egg, oats

5/9 Still seeing blood specks

5/4 Collected stool samples tested borderline for calprotectin at 68 and negative GI PCR

All her stools have a decent amount of mucus as well, more than she had before this all started.

Small rant, I was referred to a pediatric GI and after I said I cut dairy for a week and still saw the blood, he handed me a can of Nutramigen and said to stop breastfeeding. I asked about waiting longer, he said give it one extra week if no change stop breastfeeding. I asked about cutting soy, he said, Good point cut that too and give it a week and if no change stop breastfeeding and give the Nutramigen and make a follow-up appointment on your way out for two months from now.

My pediatrician is onboard with me continuing to eliminate potential triggers. I do think I cut soy too quickly and should have waited dairy out longer.

  1. Dumb question, if soy is a trigger, should I just eat tofu with miso soup and see if she reacts to know for sure?

  2. Baby girl is otherwise staying on her growth curve (85% for height) and is a very smiley and happy baby with no other symptoms. I've read if that's the case, the very small blood specks aren't even much of a concern to warrant excluding things from my diet?

  3. The week of 4/19 right before this all started we had a stomach bug involving diarrhea run through the family and we thought baby girl got by unscathed. Maybe this is how it affected her by irritating her system? Right afterwards we all came down with a cold and she's had a ton of mucus which I've read also irritates their digestive tract as well.

Our older child definitely had CMPI but could tolerate it in breastmilk, just not formula.

Any insight would be super helpful!!! I've included a picture of her most recent diaper!
