This has been a pernicious issue that I finally found the answer to, and there doesn't seem to be much good information out there. Sharing here to help out future editors feeling the pain.

In short, the problem is overlapping audio signals so premier cannot draw the waveform for each track. To fix, open the track mixer and make sure each signal is isolated in the stereo pair. If you see any number thats no 100% L or R, that's the problem.

Example of correct routing:

track 1 - stereo 1-2 - 100%L

track 2 - stereo 1-2 - 100%R
track 3 - stereo 3-4 - 100%L
track 4 - stereo 3-4 - 100%R
