Needle piercing through cap?

I normally don’t recap my Humira syringe, but today I happened to, and with very little pressure, the needle went through the sidewall of the cap and pierced my finger. Has this happened to anyone? Should I be reporting this?

US Humira - Product of Singapore?

I noticed that my newest boxes of Humira say "Product of Singapore" where previously there was nothing noted in that spot, so presumably US production.

Anyone seeing similar within the US? Any concerns about manufacturing quality now that it's in Singapore, at a newer facility?
