Hello po paano po kaya pag nakawala ako ng claiming stub na need ipasa sa windows 2 15th floor? I need to claim a document po kasi, yung ctc ng enrollment ko and I lost yung ibibigay na paper para ma claim ko siya🥲. But I have a picture naman nung stub. Paano ko po kaya siya maclaclaim? Is it possible na ipakita ko nalang po yung ID ko?

I need a health card for my pre-employment requirements.

I already have my results from other clinics, "Sanda Diagnostics Center" for x-ray, urinalysis, and fecalysis and sa "Drug Check Cubao" for my drug check. Is it possible po na i-pasa ko nalang yung results ko dun for my health certificate sa city hall? Since, based on my research, DOH accredited naman po yung clinics nila and if I'm not mistaken, tumatanggap po sila ng result from DOH-accredited clinics? Or do I really have to take another medical examination?

I also have my 1st-time job seeker certificate. Can I use that to waive the fee of the health certificate and occupational permit po? para maging free nalang sila?
