Be careful with the information you share!!

If you don't do it for the LEPs, for the providers, for legality or morality, at least do it because you're not stupid!!

It's really easy to create an app that collects information and ask an AI to "sound like XXXX profession" and have it call us colleagues, compis, etc. to promote, say app, while collecting whatever information they can get from you.

It's really easy to create a Discord server with voice channels and collect information from there.

It's really easy to sell private information online. Phone numbers, emails, addresses, and other personally identifiable information can have value.

Guys, they are profiting from your work. You think they're helping you, but they're milking you. They're even worse than the companies you think you're sticking it to.

So don't be stupid. Don't give free money to people who may use your work and the information you share for their own benefit, and don't use apps to discuss or process patient information unless you know who is behind them, what they do with your data, how they protect it, and whether they have the appropriate security measures and agreements in place.

Look for appropriate security certifications or attestations, such as SOC 2, HITRUST CSF, or ISO/IEC 27001, and make sure a Business Associate Agreement (BAA) is in place when applicable.

If an app claims to be HIPAA-compliant, actually check what that means and what protections they have in place. Don't just take their word for it.

Burnout will catch up to you

So after more than a year and a half doing interpretation without a pause, with 6–8 months working from Monday to Saturday and some weeks doing shifts for 10–12 hours, today I finally met my breaking point. I was having a weird sensation on half of my face, so I decided to take the day off.

Luckily, I have tomorrow and next week off, but I'm making this post for other people to be aware of early signs of burnout and to maybe take the day off tomorrow and this weekend during Juneteenth.

So if you are experiencing any of the following things I pulled from Google, it might be time to take a break and enjoy the long weekend.

  1. Mental & Emotional Shifts
  • Emotional Exhaustion: Feeling perpetually drained, overwhelmed, or like you are constantly "running on empty".
  • Loss of Motivation: Struggling to care about tasks that used to give you a sense of purpose.
  • Cynicism & Detachment: Feeling isolated, disconnected, or developing a negative attitude toward your work or personal life.
  • Mood Changes: Increased irritability, frustration, and a shorter temper with coworkers, friends, or family.
  1. Physical Warning Signs
  • Unexplained Aches: Frequent tension headaches, muscle stiffness, or recurring stomach/digestive issues.
  • Sleep Changes: Struggling to fall asleep, waking up tired, or craving quick-energy fixes (like sugary foods).
  • Reduced Immunity: Catching colds more often than usual.
  1. Behavioral Changes
  • Procrastination: Taking much longer to complete simple tasks and frequently putting things off.
  • Social Withdrawal: Pulling away from friends, family, or team activities.
  • Decreased Performance: Making uncharacteristic errors, becoming forgetful, or seeing a drop in overall productivity

If you Google the same thing, there's actually a good link to follow on WebMD.

Anyways, I wish you all a great weekend, and be safe.

The greatest English-to-English interpreter that has ever lived.

This week I got into a situation where I’d really like some feedback. I know there were more than a few things I was doing wrong during this call, but at the time they seemed like the best possible responses in the moment.

So, this started with an anesthesia intake appointment: a nurse and a female patient who used to be a nurse herself. The patient spoke English and understood it very well, but she had a very thick accent, and at times it was really hard to understand her. That became a major issue later on.

I started with my introduction, and while they were communicating, the patient kept insisting that she didn’t need me, that the nurse just couldn’t understand her, which was why they had called me in (still all in English).

We continued, but the accent got thicker, and the nurse grew impatient. She asked me to intervene more often and told me to finish the renditions.

Of course, the patient kept interrupting immediately after the nurse finished speaking and wouldn’t even let me start. At one point, she mentioned that two family members worked at the same hospital. One was irrelevant, but the other worked at the cath lab. The problem was that she wasn’t actually pronouncing “cath lab” clearly. We spent almost ten minutes trying to understand what she meant, and honestly you could put almost any letter at the beginning and it would sound like what she was saying.

At one point I asked for clarification:

she: Interpreter, is cath lab

(Still, she wasn’t really saying “cath lab.”)

Then I asked her to spell it.

Patient: “I won’t tell you how to spell it. It’s cath lab. You need to go back and study more if you don’t know that.”

I just smiled back at her. The nurse intervened and said, “We don’t understand you. It sounds like you’re saying ‘park lot.’”

The patient got impatient and said, “Ya fue, sigamos.”

Plot twist: eventually we came back to it. The nurse asked what they did there.

Patient: “They put the chuffff on here,” while touching her chest.

“EKG?” the nurse asked.

“Noooo…”

Eventually we figured out she was referring to the catheterization lab. Looking back, it’s obvious now.

So we finally moved on, and she still insisted on speaking English, which at that point led me to start repeating things in a softer tone for the nurse. So the nurse would ask something, the patient would answer before I could interpret, the nurse would complain that the patient shouldn’t interrupt until I finished, and I’d say, “One moment please,” to the nurse.

At that point I was basically taking what the nurse was saying and the patient’s immediate response, then delivering the interpretation afterward. Sometimes it was Spanish to English for the patient, sometimes English to English. (Paper note-taking definitely saved me there.)

Eventually we got to the end. They laughed and hugged each other. If there’s any lie in this story, I swear it’s not that part.

Patient: “Retired nurses are the worst patients, and I’m old too.”

(She had mentioned being a nurse many times before, and that was one of the reasons she kept trying to skip answers.)

Provider: “I know, honey. I’m getting old too.”

They both laughed and thanked me.

“Thank you for being so patient, interpreter.”

And I just said:

“Is there anything else I can do for you?”

“No.”

“Thank you for using our services. I wish you both a wonderful weekend. Bye Bye”

So… I know that in some parts I wasn’t strictly following protocol. For example, when she kept saying “she doesn’t understand me,” I wasn’t translating or repeating those comments. I also know it’s not my job to try to “fix” interactions, and by changing my tone of voice I wasn’t fully conveying the patient’s emotional state. But in the moment I thought: “The nurse can already see that she’s angry and understands that it’s because she can’t understand her, so why would I add more tension to that?”

At one point I was also using my “Argentinian card” and using my hands to stop her while at the same time raising and then lowering my tone of voice so she would stop talking over me.

Anyway, what would you have done differently? What kinds of things do you think I shouldn’t have done because they go against what an interpreter should do? How do you handle situations like this?

(And please don’t say “I would’ve hung up the call.” We hear that too often already, and I’m not looking for those kinds of comments.)

If I’ve ever criticized one of you during your vent posts, this is your chance to get back at me. I’ll read everything you think I could improve. In the meantime, I’m also hoping to summon one of the people who lurk deep below and are way smarter than me to provide some feedback.

TY, and:

I understood the assignment!

I got into a call where a sales representative was showing providers the technology we use for writing notes, so they could use it in cases where a patient might have a hearing impairment.

She asked me to write “Hi,” and then, while she was explaining how to use it, she said: they can write notes like: “Don’t eat anything past midnight.” I heard her and wrote that in Spanish (even though no one asked me to, but I thought an example would be great so they could see it working in real time).

I think it worked because the other person laughed and showed them, saying: “See? He just wrote that in Spanish. TYSM interpreter, have a great day.”

Anyways, just a fun dumb call I got today. Has anyone else had calls from people at the company that you work with?
