"I am not asking for medical advice." I know what I had then in 2019 and have been living with until present day. My question is the following words put into the recent visit notes by the exact same nurse practitioner who I saw in 2019 and in 2026, "lost to follow up". This is referenced twice in my online record, once to the actual test results and once to me as a human being. The records were lost to follow up and apparently so was I until I returned to treatment.....My question is what does this mean and is this just a legal butt cover phrase?

I pamper my skin all over and even learned to make homemade balms, soaps and the like in my teens.

I was pre-diabetic for a decade and about 2 years ago was dx with type 2. My skin gets weird. Dry patches, then acne and the water holding makes my skin hurt.
