So, I saw the millenial/gen z version of this in the earlier thread (source: https://old.reddit.com/r/anesthesiology/comments/1umosqa/this_is_why_patients_come_in_afraid_fear/ ), and thought i'd share this gem i was handed in 2013

RE: MY SURGERY

As a religious accommodation, I would like a completely silent surgery from the time I am induced for anesthesia until I am fully awake in the Recovery Room.

As another religious accommodation, I would further like my Minister with me after surgery in the Recovery Room so I can receive assists as soon as first vital signs there are done.

I do not want any sedative before, during or after surgery, specifically no benzdiazepines, derivatives of the drug diazepam (Valium), any drugs that include midazolam (Versed), alprazolam (Xanax), chlordiazepoxide (Librium), and temazepam (Restoril). Basically any drugs with generic names ending with the suffixes -lam and -pam. Same with barbiturates such as Seconal or Pentothal.

For an induction agent, I DO want propofol (trade name Diprivan) to put me under. The anesthetic gas agent used should be desflurane (Suprane) in a closed circuit, or sevoflurane (Ultrane).

Alternative induction agents i do NOT want include sodium thiopental (trade name Pentothal). I also do NOT want ketamine, etomidate, or nitrous oxide.

Opioids (such as morphine, fentanyl, Demerol, and Dilaudid) are okay during and after surgery. After surgery I would like to know if I can use ketorolac (Toradol), as I understand it works well as a pain relief.

So I finally decided to build an enduro bike (Pivot Firebird) and I have a spare eewings 175mm crankset that I was thinking of using. All the reading I've been able to do says enduro bikes benefit from shorter cranks, presumably due to pedal strikes. So will the 175mm crank be out of place on this bike, or will it be fine? Just trying to see if I need to bite the bullet and buy the 165mm eewings. I'm 6'2. Thanks!

hi, in general is the advance for travel expenses in addition to the stud fee? my understanding is the stud fee is pick of the litter or price of a puppy. so say they sell for $3000, do you get your expenses paid on top of that, or is it deducted form the $3k? what if you take the pick of the litter? are you responsible for expenses?

is there a standard stud breeding contract out there that i could take a look at? i'll make sure my lawyer agrees and says all provisions are enforcable but it would be nice to see what's normal language.

in the past i've only bred with people i co-own or have a close relationship with and took the pick of the litter, but i have two stud breedings planned, one is localish so i'll need reimbursement for a hotel night, food and travel expenses (is the standard to use the 72.5 cent IRS travel rate?) and the other is overseas. that's more complicated since i fly private i'm not expecting them to pay for that and i plan to go sailing afterwards, but i do expect for one night at the hotel and $100 per diem for a day but i don't know if this goes on top of the stud fee or not? does all that sound reasonable? am i forgetting anything?

lastly, is it customary to take the fee when the puppies are born or once the puppies are sold? i've heard people argue for both...

what are circumstances where you'd charge more than pick of the litter, if any? let me know if i'm forgetting anything! thanks
