When do you stop trying rescue meds & go to the er?

On day 4-5 of a migraine.

Been taking my preventative (30mg atogepant) every morning for weeks which is the most consistent ive been in a long time. Have had to take rescue meds basically everyday of this multiple times (gabapentin always, then naproxen + tylenol or advil migraine).

Been hit with nausea from smallest amount of fluids or crackers. Have thrown up a few times.

I’ve been trying to rally and get on with my days. Sometimes i wake ok and it hits worse once all meds aren’t working anymore. Other symptoms: light sensitivity, the skin around my eyes/forehead feels tight(?), overheating, neck is sore, if i press down on my left temple it radiates the migraine pain.

So at what point do you usually go in for a migraine cocktail? I’m around an 8/10. I have an extremely high pain tolerance. But i don’t want them to think i’m overreacting.

Cyclical Worsening Migraines?

I’m on atogepant for over a year. Last time i had to get my coverage renewed (not covered regularly by my drug benefits), they tried to deny because i hadn’t had enough improvement in migraine days.

I’ve noticed a pattern now where i experience 2-3 migraines a month from mid April to end of June. Each migraine lasts 3-5+ days.

Do you think I could have them increase the dose just in the summer?
