Dual Citizenship Application Question- Name change due to marriage

Hello,

I recently became naturalized US citizen and now applying for PH dual citizenship. I am married and therefore my name is different now from my birth certificate name. Do I need to submit an affidavit of explanation for the name change, if so do I need to have it notarized beforehand?

Hello,

I am posting here in case someone has similar experience with us. As the title says, my daughter’s passport application has been denied until we give more proof that her name is what we put in the application. They are rejecting the birth certificate because one of the letters in her first name does not appear as clearly printed. Say her name is KRISTINE, in the birth certificate there was a vertical line where the ink did not print properly so it looks like her name is KRIST NE. You can see the I but it was very very faint and same goes for all the areas where that vertical
line goes through.

They wanted us to submit three additional public record documents with both her place of birth AND date of birth AND her name as we put it in the application, but I am just having a hard time finding all of those information reflecting in a document. She does not have school records because she is only two and all of her medical check up would only have her date of birth. Our only hope is to find her hosital birth certificate or whatever records we could get for when she was born. Even with those, we would only have two and they require three.

Any advice from anyone who had the same situation?
