High myopee and glaucoma suspect

I’m a high myope (around −11 in both eyes). My latest OCT flagged the RNFL as borderline to outside normal limits, and the minimum rim width came back outside limits too. IOP has been steady though, usually sitting at 19 or 20.

The problem is my doctors can’t tell me what it actually means. They keep saying the myopia makes the scans unreliable, so they don’t know if there’s real glaucoma damage or if the machine is just penalising me for having a long, stretched eye it wasn’t built to measure.

So now I’m a bit in limbo. Anyone been through this? When the OCT looks off but everyone blames the myopia, what actually settles the question? Visual fields, repeat scans over a few years, something else? Mostly I just want a sense of how worried I should be.

Also have been evaluating the EVO ICL anything that I need to consider?

Evo ICL and MGD

Hey guys,

I’m looking for people who’ve had an EVO ICL after being diagnosed with myopic glaucoma. My severe myopia makes glasses uncomfortable and unsuitable for me. I’m also 24 and want to use my lenses while socialising but it’s so annoying to plan every moment to avoid irritating my eyes. I’ve already had four IPL sessions with expression and feel some relief but it’s nowhere near where I was before my diagnosis. I’m glad I still have over 70% of my glands but it’s still not comfortable. In short, I’m evaluating the EVO ICL but I want to avoid any dryness or complications and I’d like to know if it’s compatible with my condition.

abruppt onset dry eyes 9 months ago, barely any improvement despite treatment. What am I missing?

24M. Had a chalazion a few months before everything started. Then around August/September my eyes suddenly went downhill fast: multiple styes, swollen lids, extremely dry eyes. Went from wearing contacts daily with zero issues to not being able to wear them at all.

Current numbers: 25-28% meibomian gland loss lower lids (mild), NIKBUT around 5 seconds, tear meniscus 0.21mm borderline low. Meibum still comes out slightly thickened and hardened on expression. Upper lids were never imaged. Schirmer, staining, osmolarity never done. Diagnosed as ocular rosacea/MGD.

Treatment so far:Had one expression months before starting IPL at a different doctor, then one more expression combined with the first of 4 IPL sessions. So 2 expressions total and 4 IPL over the course of many months. Currently on doxy 40mg. Eyes actually got worse after the 4th IPL 3 weeks ago. No cyclosporine. No other anti-inflammatory.

I feel like 2 expressions in 9 months is way too little for obstructive MGD. The glands are still there and still producing oil, just bad quality oil. But nothing is really improving and I am stuck.

Has anyone been in a similar situation? What finally made a difference? Feeling like I am missing something obvious.

Side note: I am on finasteride 0.5mg (hair loss) which I know can affect meibomian glands through androgen suppression. Also used a topical retinoid (tretinoin for years and tazarotene that nuked my face and caused significant dryness) on my face around the time this all started, which I have since stopped. I am still a student but I am prepared to pay a lot of money to get back near my baseline. I am evaluating doing more ipls paired with expression since I believe that I have wasted money by not doing so and listening to my doctors advice. However, I am willing to only pay if it makes sense since I know that some drs push for a lot of treatments that are useless..
