Phenobarbital almost killed my dog but now I can't control her seizures

My dog(10yo F great Pyrenees mix) had 2 seizures in April and was put on phenobarbital, they think it was due to the bravecto lowering her threshold. My other dog(14yo Chihuahua mix) passed away due to cancer, and what I assumed was grief bad enough to warrant a vet visit turned into hospitalization for pancytopenia caused by the phenobarbital. She was very close to needing a transfusion but after 3 days in the hospital her numbers went up enough they thought it was safe to send her home. While in the hospital she did have a seizure (edit to add they also immediately took her off phenobarbital) which they started her on keppra 1000mg 3x a day. We get her home continue the medication without a single seizure and followed up the next week for bloodwork to make sure she's cleared from the anemia and she was great (minus a low white blood cell count) which we have a follow up Thursday to check. During that visit the vet said she could switch to keppra xr to make timing easier and while I let her know I was nervous to switch meds when these seemed to be working I went ahead and got my girl on the XR and it has been multiple seizures in the past 2 days. The clinic said to bring her in for hospitalization but I just spent $14000 there prior to the med change and I literally cannot afford to bring her back in. They keep telling me to dose her with more keppra xr and it seems her seizures just get more frequent after each dose. She's eating drinking and having no stomach issues. She acts her normal self between the seizures but it seems the keppra xr is the issue because when told to use the instant for the breakthrough seizure the first time after starting the xr it actually held them off longer. She started the XR at 1500mg twice a day and now they have her on 2250mg XR twice a day and any time she has a breakthrough seizure. Not the instant. I haven't heard back after the technician took all my information this morning so I'm hoping someone here can ease my mind in the meantime.

WTF is this?!

My entire shoal of freshwater red eye red tail puffers passed from some mystery illness after I added 4 new females to the already established 8 male/female pairs.

To preface: when I say my parameters are fine or within range- ammonia 0ppm, nitrite 0ppm, nitrate ~5ppm all using the API master kit. The pH was within 6.8-7. RO water was used for water changes.

The new fish were quarantined in a separate established tank for over 3 weeks while getting dewormed and all the things you should do when getting a new fish, especially a pufferfish. No signs of any illness, all happy, healthy, and gaining weight beautifully before I added them to the main tank with the others.

About 2 weeks together I started noticing some of their fins were turning white(no spots or anything super telling), checked parameters, everything was fine but I did a water change anyways. Waited a couple days and noticed splotches of skin fading on some of the fish both new and old fish. Immediately pulled them out and into the hospital tank for fear of epistylis or columnaris infection. Checked parameters, everything in safe range again in both tanks. Removed heat and dosed with aquarium salt in the hospital tank. The fish died within a week if not a couple days and that was after a water change and starting to dose kanaplex.

I do another weekly water change in the main tank and fish start getting sick again. Pull them out to the hospital tank, start treatment and they die. I even got so desperate on one fish I did a plastic shoebox with an air stone and methylene blue doing daily water changes. While he did survive the longest, he finally passed.

Finally I'm down to 3 fish. One male gets weird growth on his face, i moved him to the hospital tank to try to keep the remaining pair separate from whatever this disease is. This male has been living in the hospital tank for over a week now with minimal disease progression compared to the others but whatever he has looks different than what they have been dying from. He's the fish pictured. I've done nothing to the tank, I honestly moved him over expecting him to be dead within a couple of days. He won't eat but it seems any attempts at treatment are killing them so I haven't done anything to that tank just to see how he fares. I really don't know what to do.

My last female passed this morning after a water change 2 days ago in the main tank-just weekly maintenance. I left her noticing her fins turning white because whatever she had looked like what everyone else was getting, not what the one in the hospital tank has so I didn't want to get him sicker by adding her. The remaining male in the main tank seems fine for now but he hides a lot being all alone. I'm using RO water for water changes and I know the water is fine since my pea puffers haven't had a single issue under the same water change routine using the same water which is also used for the fish at my job who also have had no mortalities.

Tldr: I obviously introduced some disease with the new addition of fish but no one gets sick until I do water changes. Please help me figure out what is wrong! I have 2 males left living in separate tanks.
