I believe the PS+ last chance to play list is usually only available on the console stores, but you can actually view it on the web and mobile stores too if you know the category URL. So here are the links for the different regions.

The specific link will take you to that countries last chance list, or use the generic link to automatically redirect to your countries list (just make sure to pick the correct region for your account, otherwise you'll only see a partial list of games that are leaving).

A new update for the PS4 and PS5 versions of the game has been uploaded to the Sony update servers, it's currently in a pre-release state but hopefully should be available soon (possibly later today or early next week).
