THE 2026 PHILIPPINE NATIONAL IMMUNIZATION PROGRAM (NIP)
1. THE NEWBORN BUNDLE
BCG: 0.05 mL (Intradermal) at birth. Provides protection against miliary TB and TB meningitis.
Hepatitis B: Birth dose within 24 hours.
Note: If the mother is HBsAg positive, give the Hep B vaccine AND HBIG within 12 hours of life.
2. THE INFANT SERIES (6, 10, 14 WEEKS)
- Pentavalent Vaccine: DPT-HepB-HiB (3 doses, IM).
- Oral Polio Vaccine (OPV): 3 doses (2 drops, PO).
- Inactivated Polio Vaccine (IPV): 2 doses. The NIP now emphasizes the shift toward IPV to prevent Vaccine-Derived Poliovirus (VDPV). Usually given at 14 weeks and 9 months.
- Pneumococcal Conjugate Vaccine (PCV): 3 doses (6, 10, 14 weeks).
3. THE MEASLES PROTECTION
- 9 Months: MR (Measles-Rubella).
- 1 Year (12-15 Months): MMR (Measles-Mumps-Rubella).
- In the Philippines, due to frequent outbreaks, "Supplemental Immunization Activities" (SIA) often give MR doses as early as 6 months (the "zero dose"), but this does NOT count toward the regular schedule.
4. SPECIAL SCHOOL-BASED IMMUNIZATION
- Grade 1 & 7: Td (Tetanus-diphtheria) and MR boosters.
- HPV: Given to female students (9-14 years old) in two doses to prevent cervical cancer.
BOARD TRAP: Which vaccine is contraindicated in a child with a history of intussusception? Rotavirus vaccine. Also, remember that live vaccines (BCG, OPV, MMR, Varicella) are generally contraindicated in severely immunocompromised patients and pregnant women.

THE DENGUE SPECTRUM: PHASES AND WARNING SIGNS
1. THE THREE CLINICAL PHASES
FEBRILE PHASE (Days 1 to 3)
- Characterized by high grade fever, facial flushing, and biphasic (saddleback) fever patterns.
- Key Lab: Leukopenia (WBC drop) is often the earliest hematologic sign of infection.
- Board Trap: Do NOT give Aspirin or NSAIDs like Ibuprofen or Mefenamic Acid. These increase the risk of Reye Syndrome and aggravate bleeding tendencies. Use Paracetamol only.
CRITICAL PHASE (Days 3 to 7)
- This phase begins at defervescence, which is the period when the fever subsides.
- This is the window of Plasma Leakage.
- Key Lab: Look for a sudden rise in Hematocrit (hemoconcentration) and a rapid drop in Platelet count.
- Integrated Reasoning: Assess for signs of Third Spacing such as pleural effusion or ascites through physical examination or ultrasound.
RECOVERY PHASE (Days 7 to 10)
- During this phase, fluid is reabsorbed from the extravascular space.
- Classic Sign: The Herman's Rash, described as isles of white in a sea of red. This is a late onset maculopapular rash that is often pruritic.
- Board Trap: Monitor for Fluid Overload during this stage if intravenous fluids were not tapered appropriately when the patient transitioned from the critical phase.
2. THE 7 WARNING SIGNS (THE MUST KNOW LIST)
If a patient exhibits any of the following, they are classified as Dengue with Warning Signs and require immediate admission:
- Abdominal pain or tenderness.
- Persistent vomiting (at least 3 times in 24 hours).
- Clinical fluid accumulation (ascites, pleural effusion).
- Mucosal bleeding (epistaxis, gum bleeding).
- Lethargy or restlessness.
- Liver enlargement (greater than 2cm).
- Hematocrit increase with rapid platelet decrease.
Mnemonic: Always Practice Careful Medical Learning Like Heroes.
3. FLUID MANAGEMENT PEARLS
- Choice of Fluid: Use Isotonic Crystalloids such as 0.9% NaCl or Lactated Ringer's solution.
- Avoid: Hypotonic solutions like D5W or 0.45% NaCl should be avoided as they can worsen edema and contribute to cerebral swelling.
- Transfusion: Platelet transfusion is not routinely recommended regardless of the count unless there is significant clinical bleeding. Prophylactic transfusion does not prevent hemorrhage and may lead to fluid overload.
