Currently applying for a bursary for UCAT and I am applying since my family receives universal credit, but on the screenshot its my moms name (We have different last names) and I don't know if thats okay or do I need to submit any extra evidence?

I just found out about the new "Guaranteed Interview" system Bristol implemented from this year onwards where if you receive free school meals you get a "guaranteed" interview, and on the website it says that you need to hit a certain threshold for UCAT but I cannot find any data about the said threshold at all, has anyone applied like this this year that knows anything about this?
