AI mistakes in GRC

I've been experimenting with AI for day-to-day GRC work, and the results have been surprisingly mixed.

It can finish a security questionnaire in minutes, summarize a 100-page policy almost instantly, and even suggest control mappings across multiple frameworks. Then, out of nowhere, it'll confidently invent a regulation that doesn't exist or misinterpret a requirement. That inconsistency makes it incredibly powerful and slightly dangerous at the same time. But humans in the loop are done by most companies and some companies were working on AI only instead of training human..

What's the biggest AI mistake you've seen in GRC or compliance work that made you stop and double-check everything?

If your in-house LLM hallucinates during an audit, who is actually responsible?

I've been wondering about this lately. Many organizations are building in-house LLMs for GRC to answer security questionnaires, map compliance controls, generate policies, and support audit preparation. Keeping everything on-premises helps with data privacy, but it doesn't solve the biggest problem: a confident hallucination can still end up in an audit report or customer response if no one catches it.

The more capable these systems become, the more people are likely to trust them. That creates an interesting trade-off. If an AI-generated answer helps close deals faster but occasionally invents evidence or misinterprets a control, the financial and compliance impact could outweigh the productivity gains. At that point, is the technology truly ready for critical GRC work, or are we becoming overconfident because the responses sound convincing?

I've been thinking about something that doesn't get discussed enough.

Everyone is talking about AI replacing workflows, but what happens when an LLM confidently hallucinates in a production environment?

Imagine an AI generating incorrect financial reports, compliance advice, customer responses, security questionnaire answers, or analytics. If no one catches it immediately, even 10–15 minutes of bad output could lead to lost revenue, customer churn, SLA violations, or regulatory issues.
