Should I do the lap?

Turning 39 in November. Three failed transfer of untested embryos. Normal SIS/HSG. Provider is recommending a hysteroscpy and biopsy. Surgeon suggested doing a lap at the same time. Surgeon is a robotic gynecologic surgeon and uses both excision and ablation. I can't find anything on him on Nancy's Nook, and the fact that he uses ablation has me anxious, but he's assured me he won't touch my ovaries. I have two more untested embryos left.

Why would our clinic be against hysteroscopies?

I’m at the Albany CNY clinic and requested a hysteroscopy after my first failed transfer just for good measures. I’ve had two normal SIS and HSGs over three years. My provider cited these as a reason not to get a hysteroscopy done just yet. After my third failed transfer I brought it up again to a nurse and they said that the doctors at this clinic never do them and don’t really believe in them. Has anyone else encountered this? Ive reached out to another CNY location and am hoping to get on the schedule there, but I’m curious if others have encountered reservations around doing a hysteroscopy or know why a doctor might not “believe” in them?

Just. Keep. Transferring.

Thats what my doctor is saying after a third failed transfer of a high grade UNTESTED embryo.

I have DOR and my husband has MFI. I'm 38.5 at retrieval with an AMH around 0.45. I had 6 eggs retrieved from 7 follicles, and all 6 made it to blast.

Transfers so far:

  • 4AA: failed fresh transfer
  • 4AA: very low chemical (peak hCG 5.7)
  • 4AB: failed FET

Remaining embryos:

  • 4AA
  • 3AB
  • 3CC

Workup has been reassuring overall:

  • Normal HSG (2023)
  • Normal saline sonograms (2023 and 2026)
  • Normal thyroid and prolactin
  • Good lining and progesterone for all transfers
  • Straightforward transfers

I do have painful first day periods and some bowel symptoms during my period, so endometriosis is possible, but I've never been diagnosed and I haven't done laparoscopy, Lupron suppression, or other endometriosis treatment, and I’ve talked at length with both my doctor and a skilled lap surgeon and both are skeptical of the role endo treatments have as a means to a live birth.

What I'm really hoping to hear is from people who had a similar history and simply kept transferring untested embryos—without treating suspected endometriosis first—and eventually had success.

I completely understand that endometriosis can affect fertility, but I'm specifically looking for experiences from people who chose (or whose RE recommended) continuing transfers rather than assuming endo was the reason after 2–3 failed transfers of untested embryos.

*Please do not come in the comments demanding I advocate for myself or with stories of endo treatment being the thing that finally did it for you. I have heard so many of those stories already. I get it. I just desperately want to hear the other side.*

High aneuploid rate despite good grading?

We are transferring untested embryos and just confirmed our third failed. We’ve transferred two 4AAs and a 4AB embryo all day 5. I was 38.5 at retrieval. Curious for those who did test if they saw a high aneuploid rate with embryos of this quality or higher, especially if you were 39 or older at retrieval. If so, did anyone go on to have success with lower grades? I have a 3AB, 3CC and a day 6 4 AA left. I know it’s supposedly a beauty pageant with grading and I also know we are expected to have high chromosomal abnormality at our age but we retrieved more than anyone expected, they all fertilized, they all made it to blast, and our doctor was very hopeful given our grades. We really thought that would translate to early success.

Protocol review for untreated endo

Okay I won’t have my endo test results back in time before my next transfer. I suspect it will be positive and want to do as much as I can this transfer with that in mind. Looking for advice from those who had success with *untreated* endo—no suppression, no lap—how does this protocol look and would you add/anything differently? I’m four weeks out from transfer

Current supplements/medications:

Prenatal vitamin
C0Q10
Vitamin D
Omega-3
NAC
Baby aspirin
Magnesium Glycenate
LDN
Doxycycline (two weeks starting yesterday
Oral probiotic
Vaginal probiotic (after doxy)
Cucumerin
Resveretrol
ALA

Added the week of trigger: Pepcid, Claritin, Prednisone, and then obviously Progesterone

This will be a modified natural transfer. Positive energy only please 🤞❤️

Also cut out gluten, tracking glucose spikes, and aiming for 80-100 grams of protein, plus seed cycling.

Medical burnout and testing reservations: how and when did you say “no”?

For those who were hesitant about IVF, Western medicine, or medical interventions in general

How did you decide where to draw the line on additional testing, procedures, and treatments

How did you make peace with saying no to interventions your doctor thought might improve your odds

Did anyone pursue alternative protocols or take a more minimalist approach after IVF setbacks?

A little background: It took me years to get comfortable with fertility treatment. After an IUI and then IVF, I've had one failed transfer and one chemical pregnancy with untested embryos. My clinic is now recommending more investigation (endometriosis, endometritis, etc.).

The problem is that I'm almost 39 with an AMH of 0.4, and the potential treatments honestly scare me more than the testing itself. I have a history of anxiety and chronic joint pain, both of which I know suppression can exacerbate, and I'm not sure I would pursue suppression or surgery even if something were found.

Part of me wants to transfer the embryos I have left (3 good-quality, 1 poor-quality) without more testing and accept whatever happens. But I'm struggling with the tension between my desire for a baby and my reservations about going further down the intervention rabbit hole.

Would love to hear how others navigated that balance.

Ferritin and Iron pre-transfer...

Does anyone know why some clinics don't check ferritin numbers? I have a history of anemia, and given all the labs they run and ferritin’s role in implantation, I don’t understand why this isn’t checked. Did anyone have suboptimal ferritin and have successful transfer? I've brought mine up from 11 to 17 but everything I've read online says it should ideally be above 50. My clinic doesn't seem concerned.

Has anyone had success without a lap or suppression? My RE is incredibly skeptical of the Receptiva test and both surgery and suppressions role in improving live birth rates for women with endo. He is convinced it’s a numbers game and we just have to transfer our way through this until the right embryo sticks. I suggested maybe trying baby aspirin, LDN, and a two-week course of doxy* to treat general inflammation and infection empirically. He agreed to that. I’m curious what other treatments, protocols, or general approaches you’ve encountered that have given you success. I’m so tired
