Boss is irradiated and would have likely died from radiation, Big Boss was Irradiated TWICE and would probably have gotten super cancer if not for nanomachines and meat transplants

And Erect, Flaccid and Semi-hard Snake were all given sped up aging

Only Raiden doesn't have those problems because of being a cyborg but he also can't use his toes to pick up receipts from the floor so it's not worth it.
