Needle slams back into place, what’s wrong?

NOTE: these two pictures are different rolls but both Tri-x 400 @ 1600.
Yesterday, I developed two rolls (second slide with sleeved negatives) and the rolls came out perfectly fine. Today I developed a single roll with the same times, temperatures, dilution and the roll came out like this, utter shit. The beginning of the roll is barely developed.
Another day, another problem… this is getting extremely old to me. I have messed up 5+ rolls of very good work at this point and it’s getting extremely exhausting. Im following things by the book, yet getting shit results, or very inconsistent results.
Why has this roll developed like this?
DEV NOTES:
Tri- X 400 @ 1600
Xtol developer 1:1
Agitation for first 30 seconds and 10 seconds/minute thereafter
Chemicals were all at 78 degrees
Kodak says develop for 10:30 at 75 degrees so I took 15% off that time since chemicals were at 78 degrees and developed for 9 minutes.

Hi! I have not needed to dilute before until now, I’m developing black and white my temps for my chemicals are currently at 77 degrees so I’d like to dilute Xtol to get longer dev times. If I’m diluting 1:1 and the jobo tank needs 485ml of chemical, do I do 242.5 ML of chemical and 242.5ML of water? Or do I do 485ml of chemical and 485ml of water… stupid question probably but I’m just making sure.

This was my first time pushing film and I thought I was doing the right thing here. I’m pretty upset as I shot and dev’d 6 rolls cus I thought I had a clear understanding I set my cameras ISO to 1600 with Tri x 400 loaded and metered for the shadows in every photo. Most of the time I was at f/11 @ 250 or f/16 in bright daylight so I don’t understand why these are underexposed my cameras meter was reading accurate exposure and even overexposed on a lot of images. Am I not understanding how pushing works? Did I underdevelop? Any help is appreciated as I need to not let this happen again cus there was too much good work on these rolls.
Developing:
My chemicals were at room temps of 75 degrees and I developed using stock Xtol for 7 mins with agitation for first 30 seconds and then every 10 seconds for each minute. The digital truth site stated 7 mins was the time since temps were 75 degrees.
Just curious why they are recommending two developing times… one for 9 minutes and another for 9 minutes and 45 seconds. Is there a major difference developing for an extra 45 seconds? Is there more contrast? Thanks in advance for the help!
Finally set up my non-wet darkroom after realizing I cannot work out of my house/bathroom cus I have no room lol… so I rented a small artist studio… any tips on keeping the dust down? Thanks!
