Urinary urgency still happening after Botox? Pelvic floor mechanics might still matter

One thing I think gets missed a lot in urgency discussions is that urination is not just a bladder issue. It’s a coordination issue between the bladder, pelvic floor, nervous system, breathing system, and brain.
Normally, the bladder slowly fills while the pelvic floor stays relaxed but supportive. When it’s actually time to pee:
• The bladder muscle contracts
• The pelvic floor relaxes and drops
• The nervous system shifts into a safe “release” state
But with chronic urgency, many people unknowingly develop a constant guarding pattern:
• Clenching pelvic floor muscles
• Tight lower abs or glutes
• Stress breathing
• “Just in case” peeing
• Hyperfocusing on bladder sensations
Over time, the nervous system can start interpreting normal bladder filling as an emergency.
Botox can help by relaxing the bladder muscle itself and reducing involuntary contractions. For some people it’s life changing.
But Botox does not directly treat:
• Pelvic floor tension
• Nervous system sensitization
• Fear based urgency habits
• Constipation or bowel pressure
• Hip or abdominal guarding patterns
So sometimes urgency improves only partially because the pelvic floor and nervous system are still acting like the bladder is under threat.
A few things pelvic floor PT often focuses on:
• Learning pelvic floor relaxation instead of constant squeezing
• Diaphragmatic breathing
• Bladder retraining and urge suppression strategies
• Reducing abdominal and hip tension
• Nervous system downtraining
A few tips that sometimes help:
• Avoid “just in case” peeing every 20 minutes
• Exhale and relax the belly/pelvic floor when urgency spikes instead of clenching
• Check if constipation or straining is contributing to bladder pressure
