This is a water propogation and the roots seem to only go up? Not sure when to pot

This has slowly appeared over many months

Any help identifying this plant? Also any tips regarding care very welcome!

Little timeline progression here, this guy is suffering slowly and I'm not sure what to do, I water when the soil is drying out and gets lots of light from a westerly facing window, although it is kept on the room and not on any windowsills/in the sun.
