3 weeks on Prozac 40mg - Severe anhedonia and constant existential anxiety.

Hi everyone,

I’m looking for some insights or shared experiences from anyone who has been on a similar combination or dosage.

A month ago, I was on Prozac 20mg, but my psychiatrist increased my dose to 40mg because I wasn't seeing any improvement. I have also been taking Olanzapine 5mg alongside it. It has now been 3 weeks (21 days) on this increased 40mg dosage.

My main struggle is a deep, constant existential anxiety : an overwhelming dread about the very concept of existing, living, and being aware of my own presence.

On top of that, I am experiencing severe anhedonia. I feel absolutely no pleasure in any activity, have zero appetite, and have no desire to live or exist. My only constant wish right now is just to be unconscious and sleep to escape it all. It’s deeply exhausting.

I'm also experiencing a strange and painful paradox : while I am terrified of my own existence, I simultaneously feel completely disconnected from life and detached from reality, like I’m not really here (derealization/depersonalization maybe ?). It feels like my brain is shutting down to protect itself from the anxiety, making me feel like a ghost in my own body.

It’s been 21 days on this exact dosage, but I’m still feeling very anxious, empty, and detached. I know antidepressants take time to build up, but waiting while feeling this level of distress is extremely difficult.

For those who have taken Prozac (or the Prozac + Olanzapine combination) :

- How long did it take for you to feel a real shift in anxiety, anhedonia, and existential dread ?

- Did improvement come gradually, or did it feel like a sudden change ?

- Is it normal to still feel this level of emptiness, derealization, and anxiety 3 weeks after stepping up to 40mg ?

- At what point should I consider talking to my doctor about switching treatments ? Could Prozac simply not be the right fit for me ?

I need advice

Hey, i'm a 22M and i'm struggling with a heavy cycle of relationship anxiety, severe depression, and constant self-loathing, and I really need some outside advice. For context, I was recently diagnosed with Treatment-Resistant Depression and Generalized Anxiety Disorder. I started Fluoxetine (Prozac) two months ago at 20mg, and my psychiatrist bumped me up to 40mg a week ago. Since the increase, I’ve been living in a thick brain fog like I’m watching the world through a glass panel with constant overthinking and 24/7 anxiety. I feel no pleasure in anything, and my heart rate spikes drastically every time my phone vibrates or when I'm physically near my partner.

This anxiety stems heavily from a complicated history. I was with a girl, Y, for about two years. During a low point where I felt completely numb and desperate for happiness, I started flirting with another girl, Z. I ended things with Y to date Z, which I now realize was just me trying to escape my internal misery. Predictably, after a few months with Z, the same emotional numbness and urge to flee kicked in. I feel incapable of committing to anyone right now, regardless of how well they treat me. Eventually, I broke up with Z and tried everything to reconnect with Y, who has her own mental health struggles.

When Y and I started talking again, I felt a temporary spark of life, but things quickly spiraled. She asked if I had ever cheated or flirted with anyone while we were together, warning that it was an absolute dealbreaker. Out of fear of losing her, I lied and said no. I used to feel immense guilt specifically about that lie, but now that lie has been overshadowed by an even worse realization: I put in so much effort to get Y to talk to me again effectively disrupting her attempt to move on only to end up wanting to pull away again. I feel like an absolute monster for doing this to someone I care about.

It kills me because I desperately want to be capable of being in a healthy relationship, but I just can't handle it right now. The stress of being in this relationship is eating me alive, and I feel the urge to run away just to make the overwhelming anxiety stop. But I know that if I break up with Y now, it’s over for good. Am I just experiencing a severe, medication-induced spike in anxiety and emotional numbness that I'll regret acting on later, or should I step back from relationships entirely to focus on my mental health? Also, for those with experience on Fluoxetine, can a dose increase cause this level of intense brain fog and panic before things settle down? Any insights would mean a lot.

What's the best team i could use ?

I'm only in world 2, i've been using Wand King - Tristan dual swords - Guila rapier and Slater so far but i just pulled Elaine and since i already have King and Diane i thought it'd be a good idea to start playing with an earth team maybe ? or a wind team with Elaine as main DPS - Tristan long sword and Howzer ? but i don't know, give me advice please :)
