What is an immediate, silent red flag that tells you a person lacks basic empathy, even if they are acting polite?

At my old workplace, I really hit it off with one of my coworkers... She approached me, then we started meeting more often and there was some healthy flirting between us. This snowballed into a relationship and things were weird the first couple of months, where she'd rather spend time with her besties (most of them guys) than me, and these were people I'd been acquainted with. However, I shared my thoughts with her, which turned into a heated argument (naturally). Things changed and we started prioritizing each other more... But whenever we'd run into this someone she had feelings for, she'd openly express her excitement, such as her racing heart, and so on. On one hand, I was glad she was being honest, but on the other, I didn't want to be stuck asking myself if she'd really moved on from him. Eventually, I confronted her about that too, to which she admitted that she still had feelings for him, even though she wasn't gonna do anything about it.
Well, we aren't dating anymore because we both had to move away, but I'm still curious to understand how others would respond to a situation like this.
