Hi doctors,

I'm in a bit of a shock after a dental consultation and really need your help interpreting these scans regarding my treatment plan for tooth #14 (upper right first premolar).

I ask a second opinion from practicing implantologists and oral surgeons only please.

My primary clinic tells me that I have 10 mm of bone height and that a sinus lift is NOT required. They issued a price offer and generated a 3Shape Implant Studio surgical report, choosing a 4.5 x 8.5 mm implant.

However, when I look closely at the CT scan and their own 3Shape report, I see a major discrepancy:

  1. Two independent specialists from other clinics measured my actual, solid bone height at only 6.5 – 7 mm.
  2. There is a clear 4mm mucosal thickening (inflammatory edema) at the bottom of the maxillary sinus.
  3. It appears that the 3Shape software (and the dentist) erroneously interpreted this 4mm soft tissue edema as low-density bone (Type D3).
  4. In the cross-sectional report, the Safety Zone (green frame) of the planned 8.5mm implant is completely submerged inside the sinus cavity, right inside the edema. The apex of the implant itself is sitting directly on the sinus floor or slightly penetrating it.

The clinic defends their plan, calling it a "minimal-invasive approach" and claims a sinus lift is clinically unnecessary.

My questions to the community:

  • Is it acceptable in modern implantology to anchor an 8.5mm implant into 6.5-7mm of soft D3 bone without sinus floor elevation, essentially relying on sinus edema for "volume"?
  • What are the actual risks of immediate implant failure or chronic sinusitis if we proceed with this "zero safety margin" plan?
  • Do you agree that the software mistook the edema for D3 bone density?
