I did radically change my diet prior to starting Repatha about three weeks ago. Other than that, I've done nothing. Also of note, I have high LPa at 139 so some of that remaining LDL is LPa, but I'm hoping the repatha reduced that to some degree. Too bad about the reduced HDL but the ratio to Tri has improved a bit.
I'm simple looking for more concrete info than my surgeon gave, particularly about my cornea.
Ten days post suture removal, the 7 diopters of irregular astigmatism is gone, but on the topo map my cornea looks like it's sliding off my eye to the side. There is still some residual irregular astigmatism and when I ask if it will resolve with more time, he essentially said, that's just how RK eyes are. That's not really a prognosis either way. The good news is that i can make out the text at 20/20 but it's not really clear. the bad news is that it doesn't get better with correction. Before surgery, i had -2 diopters of corneal astigmatism, now that's gone due to the suture and corneal impacts from it, but some irregular astigmatism is still interfering. Should I expect continued changes to this cornea that result in a more regular return to prior baseline, or did the five weeks of pressure on the cornea from a suture applied when the IOP was a 6 result in a permanent remodeling. I know no one can tell me about "me", so i'm looking for information about what typically happens in similar cases.
The other issue is that i have a cloud of matter, or a single chunk of matter, floating center of my visual field from the yag. what are the odds that will eventually clear out? I'm aware for some people it never will. hopefully it's not me. Anyway, this is also interfering in the results when trying to determine correction baselines as we get geared up for LAL adjustment. I can move my eye side to side and it will clear out for a few seconds but it always drifts back. It's subtle though so I don't notice that the blur is pretty bad unless i focus on moving my eyes to clear that mess out.
Like the title says, I need a YAG and I'm only 30 days post surgery. How long do I have to wait? The OD who did my exam said I'm about a 6 out of 10 (10 being totally blind) regarding how cloudy it is already, so it's changing rapidly.
Possible confounders in that eye are that I have a suture that will need to be relieved or removed, RK scars, an ongoing PVD and I have glaucoma that hasn't affected my vision yet.
My appt with the surgeon is monday and I'm sure to find out then. But I'd still like to hear other opinions or thoughts on what I need to ask questions about regarding the laser and the PVD, or even IOP. FWIW, Google's AI said 90 days to six months. I'll be blind in that eye long before then if it continues to cloud at the current pace.
A few weeks ago I posted about possible issues with a suture in my cornea post surgery (https://www.reddit.com/r/CataractSurgery/s/gTe0InaJO5). I my initial examination today for my LAL adjustments, while I'm 20/20 in my right eye, I'm still only 20/60 with very blurry vision in my left eye. It's also uncorrectable to better than 20/60 at this time. Even the pinhole test only gets me to 20/60. They did a topographical scan of my eyes and determined that my left has 7 diopters of irregular astigmatism now. And to rub salt in the wound, I have PCO already in that same eye.
The OD is recommending the suture be removed and a YAG be performed. I'll find out whether the surgeon feels he can remove it or not, maybe someday. I have nothing scheduled as a follow up and it takes months to get a visit with this guy. At this point it's unknown when I'll be able to even see the surgeon again or if he'll be willing to remove the suture when I finally do. He was very clear when I went back complaining of vision issues from the suture that he intended it to be permanent and it was required because of the proximity to an RK incision.
I guess to sum it up, this sux.
At first I thought it was the light on the outer edge of the new lens but I see the flashes with my eye closed. So not that... then I got the floaters, but honestly, my vision has been so crappy post surgery that it's been hard to quantify. But there are some bigger ones, now. I guess you all can see where this is likely leading. I was in the clinic yesterday to evaluate why my vision is so bad and they found nothing wrong. Well now I've likely got a retinal detachment. I wish I had just not done this eye. It certainly didn't need it like the first one did.
Anyway, back to the surgeon's office I go in a couple of hours.
Edit. It was vitreous changes that suggests early PVD. Nothing to treat at this time. Better safe than sorry.
Now I have to get used to light flashes and floaters while I heal. And watch out for additional symptoms of retina issues.
I'm four days post surgery on my second eye and my vision is no better than the day after surgery. To be clear I cant tell the difference between cars or trucks riding down the road. My first eye cleared up within 36 hours of surgery. Am I just abnormally swollen in the cornea?

BTW I did call the surgeon office, nothing back yet.
Edit. I'm going in at 1030ET tomorrow for a check. I got a stitch during surgery and the surgical coordinator wants to make sure it's not caused irregular astigmatism.
As an aside, she thought it was a permanent stitch that wouldn't come out unless it was bothering me. This doesn't match anything I've read about so....
Edit. They found nothing wrong but they will remove the suture in 9 days. The OD wasn't encouraging. Since there was nothing obvious going on, she thought I might have to get used to it as is, or at least as good as LAL correction will get it. Her comment was that I just had surgery and not all outcomes are good outcomes.
It was placed because it was too close to an RK scar. I ask because it's exceptionally irritated compared to the last one that needed no stitch.
Getting my second eye done in about six hours, I started the drops yesterday morning. My right eye was pretty bad so I didn't notice what the drops did to my vision. My remaining eye had minor impacts other than some issues at night. Vision was still good quality and correctable to 20/20. After 24hrs of drops I can hardly read things are so blurry. It's no wonder you have to wait until drops are done for a final result.
There is a mild ache in general that is sometimes actual pain when I focus on something a certain way. Not sure how concerning this is at the two day mark. There is no redness and no swelling that wasn't already there yesterday. Tylenol helps a lot. I also am still on tramadol and naproxen for a rotator cuff repair a few weeks ago so that's helpful.
I'm planning to call the Dr first thing Monday if it hasn't improved. Obviously, if it gets worse, it will be the ER this weekend.
Actually, it's closer to 17 hours. Still a bit dilated so there is impact from that to my vision. I know it's early yet, but it appears that I'm even more farsighted than i was. I was a +3.25 sphere and -2.5 cyl going in, and I knew I had something like -1.5 or so in the lens so expected astigmatism until the first adjustment. What's surprising (and maybe a bit concerning) is that my glasses are still giving me better distance vision than I have without them. But everything is still blurry and a bit foggy.
I'm hoping the eye will settle down over the next week. Anyone end up with more hyperopia than they started? I'd hate for a refractive surprise to be out of the range of adjust-ability of LALs.
EDIT - I should also include that I have prior RK and PRK, so it appears I'm a challenge to calculate lens power for
Edit 2. I guess I do have a question for the sub. Has anyone see a LAL choice that was so far off that it couldn't be corrected? My understanding is that there are only two diopters of sphere and 2 of cylinder. What happens if you end up 3 diopters of sphere off?
I am a part time college teacher in retirement (state of Florida). The earnings are SS exempt supposedly, but I've never tested that since it's not a lot of money. If I was to go to full time (~$50K a year), any idea if that would impact my SS benefits? I'm 63.
