Persistent lower right low back pain for several months despite physiotherapy – looking for advice

**Age:** 27
**Sex:** Male
**Height:** 170 cm (5'7")
**Weight:** 87 kg (192 lbs)
**Country:** Germany
**Medical conditions:** Hypertension (high blood pressure)
**Current medications:** Candesartan 16 mg once daily
**Duration:** Several months

Hi everyone,

I've been dealing with persistent pain in my lower right back for several months. The pain seems to have started after I was working as a paramedic. I was sitting on a patient's bed with my back in a rounded, awkward position while inserting an IV line. I didn't feel a sudden injury at the time, but I noticed the pain afterwards, and it has persisted ever since.

The pain is mainly located in the lower right lumbar area. Sometimes it radiates into my right buttock, right leg, and occasionally toward the right side of my abdomen. It often feels muscular, but I'm not sure whether that's actually the cause.

The pain is somewhat unpredictable. Some days it becomes worse after sitting for a long time, other days after standing for a prolonged period, and sometimes after lying down for an extended time. I haven't been able to identify a consistent trigger.

The pain usually ranges from **2/10 to 5/10**. It is bothersome because it has been present for months, but it is not severe enough to prevent me from working or carrying out daily activities.

Despite the pain, I am still physically active. I continue to work as a paramedic, which involves lifting patients and carrying heavy equipment, and I also go to the gym regularly. Physical activity, exercise, and manual work are all possible for me and **do not noticeably worsen the pain**, which I find somewhat confusing.

I've already seen my primary care physician and completed a course of physiotherapy, but unfortunately my symptoms have not improved significantly.

I do **not** have:

* Fever * Unexplained weight loss * Problems with bladder or bowel control * Numbness, tingling, or other sensory disturbances * Weakness in my legs

My questions are:

* Does this sound more like a muscular issue, or could it be related to a disc, the sacroiliac joint, or something else? * Would an MRI be reasonable at this stage, given that the pain has persisted despite physiotherapy? * Is there anything specific I should ask my doctor or an orthopedic specialist to evaluate?

Thank you for taking the time to read my post. I really appreciate any advice or suggestions.

Hallo,
