Trying to understand ADA Location: Maine

So, My mental health as a bipolar person has declined recently. It also led to a relapse of alcohol after over 8 years. I really need to go to a rehab. I love my job, (especially working remote) and can't lose it (irony is I work in mental health field). Besides the communications, documentations, and keeping track of everything...what do I have to do to ensure my rights are protected under ADA? Do I fill out an application? Is it just automatically protected and my employer has to follow the law? For reference, my company has 1000's of employees nationwide. I also do not qualify for FMLA as its only been a couple months. Thank you.
