Lean bulk or cut?

5’11 about 162lbs. I’ve been stuck in the endless cut and bulk loop for 4 years (highest was 180, lowest was 150). I started out skinny fat at 150 so I bulked kind of aggressively to 180 then cut down to 165. Now I’m kind of debating if I should truly cut to abs and be skinny or lean bulk back to 175 and reassess

Palps

Obviously the rhythm and everything is fine, but I keep getting these annoying palpitations. I’ll maybe feel between 1-20 a day. And they’re usually one palp then back to normal. Occasionally a run of 3-4 if I lay down in a certain position but they stop as soon as I change positions. They feel like a jolt or jump in my stomach which spook me. My resting heart rate has also gone from high 50/low 60s to low 50s/high 40s since January but idk if it’s correlated with my cutting diet I had during the time (minimal cut, 8 lbs)?

I’ve also had upper chest pain for over 2 years now, but this occurs at random and I can run a 5k perfectly fine but then hit leg day and get the chest pain. Or even just random at my desk it’ll hurt. It’ll go away on its own or I can massage my upper back to calm it. Cardio said he’s pretty confident I’m okay, and ordered an echo.

I’ve notice I’ve been getting this flutter or drop feeling in my stomach when I breathe in occasionally. And when I get these, it’ll be like a weird sensation goes over my body. It’s not super common but does occur maybe 1-10 times a day that I feel, if at all. I’ve noticed I can occasionally get them when I run or lift too. It seems pressure on my stomach causes them? Just curious on thoughts, is this normal behavior?

Was diagnosed with INF2 gene mutation a year and a half ago, specifically c.653G>A (p.Arg218Gln). I was super blessed to find it when my egfr was at 110, some intuition with my dad having kidney disease at 25 made me check when I turned 23. I was around 800 upcr and 400 acr. (Upcr went up to 1500mg at its peak). Started losartan and upped dose slowly to 100 mg recently.

Currently sitting around 99-105 egfr and 600 upcr on my last what a month ago. My dad had a transplant at 35 but was an avid smoker and drinker + diabetes. Wanted to see if anyone knew any information about this gene or may have it themselves? Also what kind of timeline for this kind of disease can be?

Honestly can’t tell what bodyfat percentage I am currently. But I’m 160 lbs at 5’11, I know this is so small weight wise. I’m just constantly stuck when I bulk I end up fat an fluffy (around 175) then when I cut back down I gain maybe 4-5 lbs of muscle in that bulk. Been this way for 4-5 years. Debating if I should commit to a full cut and get lean or just recomp for a while

This is my heart rate pattern to today at the office. Just seems low to me? A year ago I was in the 58-65 range. Now I get to the 47-55 range. I haven’t really been more active, still lift 4 times a week and run a 5k on Saturdays (the last 4 weeks). But I’m 5’11 160lbs. I get heart palpitations occasionally, mainly when I’m either taking a deep breath, holding my breath subconsciously, or laying in bed in certain positions.

Went to the cardio and he didn’t seemed alarmed. Did the standard ekg in the office and I guess he saw nothing. He ordered an echo too, no stress test since I can run 5ks and get my heart rate up to 160 with recovery of 31 bpm.

Side note: I’m chronically tired and mentally fatigued. I’m in the office and could fall asleep at my desk if I wanted to, or even when I drive. I get 7-8 hours of sleep but it’s non restful it seems. I got cleared for apnea but I dry out in my sleep (hematocrit at 53-55% when I wake up but it drops to 47% soon after).
