Recent APW experiences???

I've gone to APW for years for my routine gynecological care and have Lisa Garcia, who is absolutely amazing. I just found out I'm pregnant (first pregnancy) and scheduled my initial appointment. I was told that none of the midwives were available and that I would have to have my 8 week appointment with an MD. Also, it will be a temporary, locum doctor because it sounds like all their doctors, except maybe 2, have either quit or are on leave since earlier this year. This kind of terrifies me (an already anxious person).

I've known for a while that I want to see a midwife based on my experiences with Lisa. I was told after the 8 week appointment that I will be able to switch my care to the midwife team. But feeling scared with how understaffed that office is and what may happen if the pregnancy turns out to be high risk.

Anyone have any experience with their locum doctors recently? I will be seeing a Dr. Blockman.

Pregnancy

I don't have a NCAH diagnosis yet but I think it is highly likely. We've been ttc for a year, which prompted me to do testing, which revealed high dhea/testosterone. And then my 17-OHP test results last month were 1797 ng/dL. I also have at least 1 gene copy based on previous carrier testing. I've been referred to an endocrinologist for further evaluation and diagnosis but the wait list is 8-10 months.

Meanwhile, just got back from vacation, period 3 days late, and pregnancy test is positive. I'm kind of shocked because after learning about these hormonal imbalances I'd kind of started to give up on the idea of getting pregnant naturally.

What I'm wondering is, is there anyone on here who's had successful pregnancies without intervention? Now I'm scared that my hormonal imbalances are going to cause a miscarriage, since I'm not able to see an endo anytime soon. I'm especially scared of a late term miscarriage. Is there anything I need to watch out for? Will my high dhea/testosterone cause developmental harm?

Worried about drinking during TWW.

Not sure if this the right place to post, but just wanted to express my fear/worry.

I just took a pregnancy test (3 days after period should have started, 17 dpo) and within 2 minutes there was a solid positive line. I'm a little shocked because we've been trying casually for a year and just a month ago I found out I have a hormonal imbalance that causes high dhea and testosterone. Even though my periods are regular I was starting to assume I wasn't ovulating and that it would be unlikely for me to get pregnant naturally.

Just got back from a 2 week vacation in Spain. Had sex the day we left, which lines up with when I should have been ovulating. I drank wine pretty much every day of this vacation, usually 1-2 glasses per day but there were a few fancy dinners where I drank probably about 4 glasses worth over the course of 2-3 hours. In fact, it was when we got back from one of these dinners that I noticed some very light/brown blood on toilet paper. This continued for 4 days. Implantation bleeding crossed my mind but I convinced myself it was probably just irregular bleeding due to long distance travel/circadian rhythm disruption (air travel has messed up my cycle before).

I feel so stupid. I have been so careful leading up to this. Had reduced my drinking overall the past year, made sure to only have 1-2 glasses during the TWW, or none at all. And now I worry that I've caused developmental harm or have just increased the possibility of miscarriage, which I'm probably already at risk for based on age (37) and hormonal imbalance.

Oh, also barely took a prenatal the past 2 weeks, even though I had been taking them everyday for 1.5 years leading up to this.

Investigating possible NCAH

37f who recently had a standard fertility panel done after no luck conceiving for the past year. Everything came back in the normal range except testosterone (total 75, free 8.5) and DHEA (892). I got the testosterone results back first which set me off on a spiral of PMOS and NCAH.

Reading about NCAH, I am very interesting in further testing as there are some things that are resonating with my life. I grew pubic/body hair at a young age, was lean/muscular in my youth without much effort. In my early 20s I started to notice some excess hair growth on my chin. My hair was thick in my teens but has slowly thinned for the past 15 years, especially the past year since going off birth control. I've also notice a mild-moderate increase in abdominal fat since going off birth control and it has bee. So much more difficult to lose any weight. Also, I did genetic carrier testing a few years ago and I am an a carrier for CAH.

I was on the pill for 8 years, then an IUD for 8 years. Now I'm thinking that being on hormonal birth control for 16 years of my adulthood has masked ncah. On the other hand, I am typical height, my acne is average and usually just increases in my luteal phase. I also have had regular periods since coming off birth control.

My DHEA number is freaking me out. My doctor was going to recommend an ovarian ultrasound but after seeing my labs she is now recommending a CT scan. Now my thoughts have shifted from ncah to a tumor. I am an highly anxious person, especially health anxiety, and I am spiraling and freaking out at the idea of a tumor or cancer.

My doctor also did order a 17-hydroxyprogesterone (17-OHP) test, which I know is one step in ncah diagnosis. I've read some great information on here but curious what other steps are needed for a diagnosis.
