Concerta XL 18mg makes me feel kinda drunk?

I got my ADHD diagnosis and got prescribed concerta xl 18mg. When i first took it for the first few hours i felt very awake, kinda like with nicotine, and had tunnel vision, and my internal dialogue was much faster and didn't stop (just like when I'm not on meds). Then after 4 hours it completely wore off.

The next few days i felt almost nothing, maybe i felt a bit different, kinda out of it, like my focus is worse.

Then I skipped a day and omg i felt so tired and felt groggy all day, i was walking outside with my eyes closed.

Today i took it again and i feel like when you're drunk or extremely tired and if you tried doing a maths/logic problem, you wouldn't be able to do it. My mind feels very clouded. My eyes are unfocused and I'm just staring at a wall.

Coffee never worked for me except make me sleepy and make my heart pound.

When i tried nicotine for the first time years ago it was like a lightbulb came on. I felt like all my terrible terrible brain fog and fatigue vanished and i could finally THINK clearly for the first time. I felt super calm bc usually im anxious 24/7. It just felt perfect and i knew i prob had adhd but i kinda kept denying it. I went to get a private autism diagnosis and the doctor did an adhd assessment for free bc he knew i wouldn't get one, i kept being like wdym i dont have adhd i have autism. Maybe he knew i would never get medicated if he wouldn't diagnose me. After a few months I've realised i totally have adhd, and quite a bad case. Very severe procrastination and brain fog and internal dialogue that legit does not stop for 1 second during the day.

I thought methylphenidate would feel like nicotine, and it felt like that on the first day, but since then I've felt the opposite. My face also has a lot of blood in it.

I'm thinking of taking a double dose bc my doc is private and an appointment is expensive, he'll prob increase my dose anyways before trying vyvanse.

I'm from the UK, we don't do Adderal.

Classical Piano Song Beginning

It's a semi-popular tune. Anyone who listens to classical/piano knows it and I'm pretty sure I've heard it on reddit videos a few times.

It starts off with one or two chords in the middle of the piano, then the melody that i just played, then the chords again, then my melody, then it starts getting louder and more serious, quite tragic and sorrowful, then calms down a bit again. That's the first 30 seconds of it.

I've been looking for it for hours but can't seem to find it.
