Mature Eggs and follicle sizes

I’m a 39 F- Has anyone figured out what follicle sizes actually give them mature eggs? I’m trying to optimize maturity for future IVF cycles and feeling confused because my results seem all over the place.

In my first retrieval, at trigger I had follicles around 21, 12, and 13 mm. They got 3 eggs total, and 2 were mature at retrieval. The third matured later through IVM. I sadly got not blasts. My trigger that cycle was 5,000 hCG and lupron

In my second retrieval, I had follicles measuring about 18, 17, 16, 16, 15, 14, and 12 mm. They retrieved 5 eggs, and 3 were mature. This time my trigger was actually much stronger (15,000 hCG) with lupron.

I stimmed for 9 days for cycle 1 and 13 days for cycle 2

The good news is that the 3 mature eggs all fertilized and ended up giving me 2 blasts, which I’m really grateful for. But I’m trying to figure out how to maximize the number of mature eggs next cycle so I have more chances at getting blasts.

Has anyone noticed that their eggs only tend to be mature at certain follicle sizes? Did changing your trigger timing, trigger dose, or waiting an extra day help improve maturity?

It just feels like maturity is so variable from cycle to cycle, and I’m wondering if there’s a way to figure out what my “sweet spot” is. Would love to hear if anyone has had a similar experiences and what helped!

Day 3 or day 5 transfers?

Hi everyone. I’m looking to hear from women who have had a similar IVF experience, especially those who were around 39 with low ovarian reserve and struggled to make blasts but eventually had success.

I’m 39 as of March and have an AMH of 0.8. My AFC is usually around 8–10. I’ve done three IVF retrievals so far:

Cycle 1 and 2- nothing made it to blast

I recently did cycle 3 and have one 3AB blastocyst, which has given me some hope, but the process has been emotionally exhausting. My doctor is recommending continuing with my IVF protocols, and I’m trying to decide whether to keep pursuing blastocysts and PGT-A testing or consider transferring earlier-stage embryos at day 3.

The thing that makes me worried is having a miscarriage and being set back but I’m seeing several folks who have success with day 3 transfers with DOR. It’s like a risk to do it and then a risk if you don’t?! I’m so torn

I’m wondering:

Did anyone else have multiple cycles with very few eggs and poor blast rates before eventually finding success?

Did anyone with low embryo numbers choose Day 3 transfers instead of waiting for blasts?

If you were over 39, how many retrievals did it take before you got a euploid embryo or a live birth?

I’d really appreciate hearing your experiences both success stories and lessons learned. I’m trying to stay hopeful while also making the best decisions moving forward. Thank you so much for reading.

Euploids and DOR

I am 39 and I have diminished ovarian reserve after two retrievals and a lot of trial and error finding the right protocol. I finally made my first blast a 3AB. We had nothing to freeze from my previous two rounds so getting to this point feels like a huge win. Now I’m in the waiting game for PGT results and my anxiety is through the roof.

I have batching selected so once I get six blasts/embryos, they can test it. I’ve read around 60% of eggs or embryos at my age are not normal but I also know statistics don’t predict what happens to one person. I’m hoping to hear from others that are 39 and over if you banked embryos how many were you euploid especially if you have DOR. It’s going to be a long journey for me!

Had a Union station trip!

I haven’t been to Union station for a bit but had to go this weekend and on our way back we stumbled upon East Street Cafe, it’s on top floor kind of hidden and not too busy and omg amazing prices and service and the food was incredible! I had drunken noodles and it was so good! I’m dying to go there again

Just wanted to share in case someone is traveling this summer or lives nearby!

Low numbers and looking for hope

Hi everyone,

I’m 39 and just had my second retrieval. This cycle, I had 5 eggs retrieved and 3 fertilized normally (2PN). I’m trying to stay hopeful but, after previous cycle that didn’t make it to blast, I’m finding the wait really hard. Last cycle I had 3 retrieved 1 fertilized with 2pn and 3pn and the other didn’t show signs of fertilization and no blasts.

I’m looking for some encouragement and would love to hear from anyone around age 39 (or late 30s/early 40s) who had similar numbers and went on to have success.

I’d especially love to hear from anyone who had only 3 normally fertilized embryos at similar age and still ended up with a good outcome. I know every cycle is different, but hearing some hopeful stories while I wait for blast results would mean a lot.

Calcium Ionphere for blasts?

I’m 39 with DOR and have had two IVF retrievals with disappointing embryo development.

In one cycle I had decent maturity and fertilization but no blasts. In another cycle I had a better-looking follicle cohort but poor maturity and again no blasts.

I’m trying to understand whether calcium ionophore helped anyone who:

had normal or near-normal fertilization

but embryos arrested before blastocyst

had repeated cycles with very low blast rates

Did your clinic recommend it? Was there evidence of an egg activation issue, or was it more of an empiric add-on after poor embryo development?

I’d love to hear:

your age/amh

fertilization rates before and after

whether blast rates improved?

Our clinic will do zymot and icsi

My Dr is like meh about it- “he does not think it is necessary but is willing to add it if you would like. He put it in your care plan as optional, please let me know if you would like to add it to your plan.”

AFC and eggs actually retrieved?

For those of you who have gone through IVF, how closely did your AFC correlate with the number of eggs retrieved?

I’m curious because my AFC is often higher than the number of eggs I ultimately end up retrieving. For example, my baseline AFC this cycle was 12, but in previous cycles where it was 8-10 and I’ve had fewer eggs retrieved than expected based on my AFC. I also didn’t prime? So my first round I had one lead over the others

Did you typically retrieve about the same number as your AFC? fewer than your AFC?or more than your AFC?

If you’re comfortable sharing, I’d love to know your age, AFC, and how many eggs were ultimately retrieved

Trouble making blasts? Age 38+

Has anyone had repeated fertilization but trouble getting embryos to blast?

I’m 39 and have diminished ovarian reserve. I’ve done two IVF retrievals so far. In both cycles, I was able to get eggs and achieve fertilization, but I haven’t been able to get a blastocyst yet.

Cycle 1: retrieved 3 eggs,3 matured, fertilization occurred in all but no blasts.

Cycle 2: retrieved 3 eggs, only 1 was mature at retrieval and matured overnight. Fertilization occurred, but again no blasts.

Unexplained for me as everything test normally my husband has excellent sperm parameters my clinic uses Zymot and ICSI, I used Omni for my second cycle, which turned out worse?

I’m trying to understand whether others have had a similar pattern, fertilization is happening, but embryos arrest before blast. If so:

Did you eventually make a blast in a later cycle?

Did you change anything that seemed to help (protocol, trigger timing, stimulation dose, supplements, sperm factors, etc.)?

Did your doctor have an explanation for why fertilization was occurring but embryos weren’t reaching blast?

I’d especially love to hear from anyone who was 38–40 with DOR and eventually had success after initially struggling to make blasts. This is so hard because my friend is 41 and her husband is 50. They just did one IVF of cycle and got 9 blasts four euploids and I’m newly 39 and my husband is newly 35 and unable to make it to blast. I don’t understand. I’m really sad.:(

Trouble making blasts

Turned 39- first round 3R/3M/3F no blasts

Second round did shortly after first was an incredibly bad round I don’t even want to count it but I had 3 retrieved and 1 matured the next day and fertilized but no blasts Second round which is typical for next day eggs

Any tips or tricks? I’ve heard a 20 hcg trigger can get eggs out of Tiny follicles? My husband has excellent sperm- I have a 100% fertilization rate on mature eggs. Should I ask him to maintain a short abstinence, we usually do a release the day I trigger maybe I should do it the day before the retrieval?

Is there anything like small things I can do to help better blast rates or is it just a numbers game and my numbers are too low to even have blasts?

I’ve seen some folks on here get blast from like 2 to 3 eggs or even one so that’s why I feel really bad. I have never gotten that milestone.
